摘　　要：目的探讨胱抑素C(Cys-C)、同型半胱氨酸(Hcy)、细胞角蛋白19片段(CYFRA21-1)在肺癌分期及预后中的价值,以达到及早确定患者肺癌分期,改善患者预后的目的。方法选取2020年12月至2022年11月安徽省胸科医院收治的104例肺癌患者作为研究对象,根据TNM分期将患者分为早期组(Ⅰ~Ⅱ期,n=43)和中晚期组(Ⅲ~Ⅳ期,n=61),根据院内28 d结局分为预后良好组(体重增加≥2kg,n=36)和预后不良组(体重增加<2 kg,n=68)。比较早期组和中晚期组的Cys-C、Hcy、CYFRA21-1水平及一般资料,采用Spearman相关性分析Cys-C、Hcy、CYFRA21-1与肺癌分期的相关性。采用多因素Logistics回归分析影响肺癌患者预后的独立危险因素,采用受试者工作曲线(ROC)分析Cys-C、Hcy、CYFRA21-1预测肺癌患者预后不良的价值。结果早期组和中晚期组的年龄、文化程度、饮酒史、吸烟史、病理类型以及血清Cys-C、Hcy、CYFRA21-1水平比较,差异具有统计学意义(χ^(2)=16.136、9.903、4.071、4.105、6.508,t=31.603、19.102、10.907,P<0.05)。Spearman相关性分析结果显示,Cys-C、Hcy、CYFRA21-1水平均与肺癌患者的分期呈正相关(r=0.642、0.597、0.613,P<0.05)。多因素Logistics回归分析显示,Cys-C、Hcy、CYFRA21-1为肺癌患者预后不良的独立危险因素(P<0.05)。ROC曲线结果显示,Cys-C、Hcy、CYFRA21-1单独检测和联合检测预测肺癌患者预后不良的曲线下面积(AUC)分别为0.842、0.761、0.755、0.916。结论Cys-C、Hcy、CYFRA21-1与肺癌分期密切相关,且Cys-C、Hcy、CYFRA21-1联合检测对肺癌患者预后不良的预测价值较高,值得临床推广使用。Objective To investigate the value of cystatin C(Cys-C),homocysteine(Hcy)and cytokeratin 19 fragment(CYFRA21-1)in the stage and prognosis of lung cancer,to determine the stage of lung cancer early and improve the prognosis of patients.Methods A total of 104 patients with lung cancer admitted to Chest Hospital of Anhui Province from December 2020 to November 2022 were selected as research objects.According to TNM staging,the patients were divided into the early group(stage Ⅰ~Ⅱ,n=43)and the late group(stage Ⅲ~Ⅳ,n=61),and divided into good prognosis group(weight gain≥2 kg,n=36)and poor prognosis group(weight gain<2 kg,n=68).The levels and general data of Cys-C,Hcy,CYFRA21-1 in the early and late groups were compared,and Spearman correlation was used to analyze the correlation between Cys-C,Hcy,CYFRA21-1 and the stage of lung cancer.Multivariate Logistics regression was used to analyze the independent risk factors affecting the prognosis of lung cancer patients,and receiver operating curve(ROC)was used to analyze the value of Cys-C,Hcy and CYFRA21-1 in predicting poor prognosis of lung cancer patients.Results There were significant differences in age,education,drinking history,smoking history,pathological types,and serum levels of Cys-C,Hcy and CYFRA21-1 between the early and late groups(χ^(2)=16.136,9.903,4.071,4.105,6.508,t=31.603,19.102,10.907,P<0.05).Spearman correlation analysis showed that the levels of Cys-C,Hcy and CYFRA21-1 were positively correlated with the staging of lung cancer patients(r=0.642,0.597,0.613,P<0.05).Multivariate Logistics regression analysis showed that Cys-C,Hcy and CYFRA21-1 were independent risk factors for poor prognosis in lung cancer patients(P<0.05).The ROC curve results showed that the area under the curve(AUC)of CYS-1,Hcy and CYFRA21-1 alone and combined detection in predicting poor prognosis of lung cancer patients were 0.842,0.761,0.755 and 0.916,respectively.Conclusion Cys-C,Hcy and CYFRA21-1 are closely related to the stage of lung cancer,and the combined detection
