摘 要:目的:探究颌间牵引诱导安氏Ⅱ类错牙合成中性或趋于中性的正畸效果及对软硬组织变化的影响。方法:选取2019年5月-2021年6月笔者医院接诊的60例安氏Ⅱ类错牙合患者为研究对象,排齐牙列、颌间牵引、橡皮链牵拉关闭间隙的过程中,上颌15、16、25、26人为地增加抵御前移的支抗治疗。观察并记录治疗前后的正畸效果和同行评估等级(Peer assessment rating,PAR)指数,检测金属蛋白酶组织抑制物-2(Tissue inhibitor of metalloproteinase-2,TIMP-2)和基质金属蛋白酶-2(Matrix metalloproteinase-2,MMP-2),对其矫正前和矫正后6个月头颅侧位片软硬组织相关指标进行测量,观察软硬组织变化情况。结果:矫治后患者PAR值(中线、牙齿排列、颊侧区咬合)、SNA角、ANB角、U1-NA角、U1-NA距、U1-FH角、ULP值较治疗前变小(P<0.05);SNB角、OP-FH角、U1-L1角、SnLs-SiLi角、Cm-Sn-Ls角、TIMP-2、MMP-2值较治疗前增大,差异有统计学意义(P<0.05)。结论:安氏Ⅱ类错牙合患者采用颌间牵引,橡皮链关闭间隙治疗,可有效改善患者上颌前突、下颌后缩的面型,矫正深覆牙合、深覆盖效果良好。Objective To explore the effect of intermaxillary traction on classⅡmalocclusion and the changes of soft and hard tissues.Methods A total of 60 patients with ClassⅡmalocclusion in the author’s hospital from May 2020 to June 2021 were selected as the research objects.In the process of aligning dentition,intermaxillary traction and rubber chain traction to close the gap,the maxillary 15,16,25 and 26 were artificially provided with anchorage therapy to resist forward movement.Following-up and using the peer assessment rating(PAR)to evaluate the orthodontic effect after treatment.Detecting the levels of tissue inhibitor of metalloproteinase-2(TIMP-2)and matrix metalloproteinase-2(MMP-2)and observing the changes of soft and hard tissues in lateral cephalograms which were shot before and after corrected.Results After treatment,the PAR(median,tooth arrangement,buccal area occlusion),the SNA angle,the ANB angle,the U1-NA angle,the U1-NA length,the U1-FH angle,and the value of ULP were less than those indexes before treatment while the the SNB angle,the OP-FH angle,the U1-L1 angle,the SnLs-SiLi angle,the Cm-Sn-Ls angle were greater than those indexes before treatment(P<0.05).Conclusion The intermaxillary traction have a good effect which can effectively correct deep overlay-and deep coverage and improve the patient's facial shape for patients with ClassⅡmalocclusion.
