无托槽隐形矫治器在儿童前牙移位矫治中的应用

摘　　要：目的:观察无托槽隐形矫治器矫正儿童前牙移位的效果。方法:选取笔者医院2019年6月-2020年6月收治的前牙移位患儿94例,随机分为传统组47例和观察组47例。传统组采用直丝弓固定矫治器矫正患儿前牙移位,观察组采用无托槽隐形矫治器矫正前牙移位。比较两组患儿矫治后的美观度、便捷性、语言功能及咀嚼功能、菌斑指数、牙龈指数、龈沟探诊深度、并发症及矫治效果。结果:观察组患儿在矫治过程中自觉美观程度、便捷程度、语言功能评分明显高于传统组(P<0.05),疼痛程度低于传统组(P<0.05);观察组患儿矫治后咬合力、咬合接触面积及咀嚼效率相较于传统组更高(P<0.05);两组患者各自佩戴12个月矫治器后菌斑指数、牙龈指数及龈沟探诊深度均较佩戴前升高(P<0.05),但观察组以上指标均低于传统组(P<0.05);随访12个月,观察组患儿出现口腔黏膜溃疡发生率和并发症总发生率明显低于传统组(P<0.05);观察组的总有效率(95.74%)与传统组(82.98%)比较差异无统计学意义(P>0.05)。结论:无托槽隐形矫治器对于儿童前牙移位具有较好的矫治效果,相较于传统的直丝弓固定矫治器,无托槽隐形矫治器更具有美观性和便捷性,减少了对患儿语言功能、咀嚼功能的不良影响且疼痛程度较低,有利于口腔清洁与牙周健康的维持。同时在佩戴过程中并发症发生率更低,值得临床推广应用。Objective To observe the effects of bracketless invisible appliance in correcting anterior teeth displacement in children.Methods 94 children with anterior teeth displacement in the author’s hospital were selected between June 2019 and June 2020,and were randomly divided into 47 cases in traditional group and 47 cases in observation group.The traditional group adopted straight wire arch fixed appliance to correct the displacement of anterior teeth,and the observation group received bracketless invisible appliance to correct the anterior teeth displacement.The aesthetics,convenience,language function,chewing function,plaque index,gingival index,gingival crevicular depth,complications and correction effects were compared between the two groups of children after treatment.Results The scores of conscious aesthetics,convenience and language function during correction process in observation group were significantly higher than those in traditional group(P<0.05),and the pain level was milder than that in traditional group(P<0.05).The occlusal force,occlusal contact area and chewing efficiency after correction were larger or higher in observation group than those in traditional group(P<0.05).The plaque index,gingival index and gingival crevicular depth in the two groups were increased after 12 months of wearing the appliance compared with those before wearing the appliance(P<0.05).But the above indicators in observation group were lower compared with those in traditional group(P<0.05).After 12 months of follow-up,the incidence rate of oral mucosal ulcers and total incidence rate of complications were significantly lower in observation group compared to traditional group(P<0.05).There was no significant difference in the total effective rate between observation group and traditional group(95.74%vs 82.98%)(P>0.05).Conclusion Bracketless invisible appliance has a better correction effect on anterior teeth displacement in children.Compared with traditional straight wire arch fixed appliance,bracketless invisible appliance
