瞬目反射及面神经运动传导测定、神经电图、面肌肌电图对面神经炎早期诊断、治疗、预后的价值评估

摘　　要：目的探讨瞬目反射及面神经运动传导测定、神经电图、面肌肌电图对面神经炎早期诊断、治疗、预后的价值。方法80例面神经炎患者作为研究对象,均接受瞬目反射及面神经运动传导测定、神经电图、面肌肌电图检查。分析瞬目反射检查结果,比较面神经运动传导测定、神经电图、面肌肌电图检查异常率及不同M波下降幅度面神经炎患者的痊愈时间。结果80例患者全部异常,其中18例患者R1、R2、R2'波缺失;23例患者R1波缺失,R2、R2'波潜伏期延长;39例患者R1、R2、R2'波潜伏期延长。患侧R1波(13.24±1.37)ms、R2波(31.84±4.52)ms高于健侧的(10.86±0.99)、(28.75±3.52)ms,R2'波(30.75±3.45)ms低于健侧的(36.56±3.43)ms,差异有统计学意义(P<0.05)。面神经运动传导测定检查异常率62.50%高于神经电图的42.50%、面肌肌电图的46.25%,差异有统计学意义(P<0.05)。神经电图与面肌肌电图检查异常率比较,差异无统计学意义(P>0.05)。M波下降幅度<50%、50%~80%以及>80%面神经炎患者的痊愈时间比较,差异有统计学意义(P<0.05)。且痊愈时间随M波下降幅度的增大而延长。结论瞬目反射及面神经运动传导测定、神经电图、面肌肌电图检查可客观评估面神经炎患者病情,其中瞬目反射是面神经炎患者早期诊断的敏感指标,面神经运动传导测定、神经电图、面肌肌电图可提示患者异常情况,可以提早确诊病情,提早进行治疗干预,利于评估面神经炎患者病情、预后。Objective To discuss the value of blink reflex,facial nerve motor conduction measurement,electroneurography and facial muscle electromyography in the early diagnosis,treatment and prognosis of facial neuritis.Methods 80 patients with facial neuritis were studied and underwent transient reflex and facial nerve motor conduction measurements,neuroelectrography,and facial electromyography.The results of the blink reflex examination were analyzed,and the abnormal rates of facial nerve motor conduction measurement,electroneurography and facial muscle electromyography were compared,as well as the healing times of patients with facial neuritis with different M-wave decreasing amplitude.Results All 80 patients were abnormal,including 18 patients with absent R1,R2,and R2'waves;23 patients with absent R1 waves and prolonged R2 and R2'wave latencies;39 patients with prolonged R1,R2,and R2'wave latencies.The R1 wave of(13.24±1.37)ms and R2 wave of(31.84±4.52)ms on the affected side were higher than those of(10.86±0.99)and(28.75±3.52)ms on the healthy side;the R2'wave of(30.75±3.45)ms on the affected side was lower than that of(36.56±3.43)ms on the healthy side;the differences were statistically significant(P<0.05).The abnormal rate of facial nerve motor conduction measurement was 62.50%,which was higher than that of neuroelectrography(42.50%)and facial electromyography(46.25%),and the difference was statistically significant(P<0.05).The difference was not statistically significant when comparing the abnormal rate of neuroelectromyography and facial electromyography examination(P>0.05).There was statistical significance in the healing time of facial neuritis among patients with M-wave decrease of<50%,50%-80%and>80%(P<0.05).The healing time was prolonged with the increase of M wave decreasing amplitude.Conclusion Blink reflex,facial nerve motor conduction measurement,electroneurography and facial muscle electromyography can objectively evaluate the condition of facial neuritis patients.Blink reflex is a sensitive indicato
