SafeGuard压迫止血装置与绷带包扎在肝癌TACE术后止血中的效果对比分析

摘　　要：目的对比分析SafeGuard压迫止血装置与绷带包扎在肝癌肝动脉化疗栓塞术(TACE)术后止血中的临床效果。方法回顾性分析行TACE治疗的肝癌患者病历资料,通过绷带包扎进行止血的患者为绷带包扎组(49例),通过SafeGuard压迫止血装置进行止血的患者为压迫止血装置组(49例)。比较两组患者止血操作时间、肢体制动时间、拆除包扎时间、卧床时间、血肿形成率、局部渗血率、皮肤瘀斑率、排尿困难率、假性动脉瘤率、皮肤受损率。结果压迫止血装置组止血操作时间(5.31±0.86)min、肢体制动时间(6.06±0.22)h、拆除包扎时间(6.19±0.41)h、卧床时间(12.07±1.23)h均短于绷带包扎组的(19.82±3.51)min、(7.85±0.56)h、(12.63±0.85)h、(23.31±1.89)h,差异均具有统计学意义(P<0.05)。压迫止血装置组血肿形成率2.04%、局部渗血率8.16%、皮肤瘀斑率2.04%均低于绷带包扎组的22.45%、30.61%、16.33%,差异具有统计学意义(P<0.05)。两组排尿困难率、假性动脉瘤率、皮肤受损率比较,差异无统计学意义(P>0.05)。结论与绷带包扎相比,在肝癌TACE术后采用SafeGuard压迫止血装置止血效果更好,可缩短止血操作时间、卧床时间等,减少血肿、局部渗血、皮肤瘀斑发生率,更加安全有效,临床使用价值高。Objective To compare and analyze the clinical effect of SafeGuard compression hemostatic device and bandage dressing after transcatheter arterial chemoembolization(TACE)for liver cancer.Methods The medical records of liver cancer patients treated with TACE were retrospectively analyzed.Patients who had hemostasis by bandage dressing were selected as the bandage dressing group(49 patients),and patients who had hemostasis by SafeGuard compression hemostasis device were selected as the compression hemostasis device group(49 patients).Comparison was made on hemostatic operation time,limb braking time,bandage removal time,bed rest time,and the rate of hematoma formation,local blood oozing rate,skin ecchymosis rate,dyuria rate,false aneurysm rate and skin damage rate between the two groups.Results In the compression hemostasis device group,the hemostatic operation time of was(5.31±0.86)min,the limb braking time was(6.06±0.22)h,the bandage removal time was(6.19±0.41)h,and the bed rest time was(12.07±1.23)h,which were shorter than those of(19.82±3.51)min,(7.85±0.56)h,(12.63±0.85)h,and(23.31±1.89)h in bandage dressing group,and the differences were statistically significant(P<0.05).In the compression hemostasis device group,the rate of hematoma formation was 2.04%,the local blood oozing rate was 8.16%,and skin ecchymosis rate was 2.04%,which were lower than those of 22.45%,30.61%,and 16.33%in the bandage dressing group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in dyuria rate,false aneurysm rate and skin damage rate between the two groups(P>0.05).Conclusion Compared with bandage dressing,SafeGuard compression hemostatic device has better hemostatic effect after TACE for liver cancer,which can shorten the hemostatic operation time and bed rest time,reduce the incidence of hematoma,local blood oozing and skin ecchymosis,and is more safe and effective,with high clinical value.
