腮腺浅叶部分切除术与腮腺浅叶切除术治疗腮腺多形性腺瘤的效果比较

摘　　要：目的探究腮腺浅叶部分切除术(PSP)与腮腺浅叶切除术(SP)在腮腺多形性腺瘤治疗中的效果。方法100例腮腺多形性腺瘤患者,根据手术方法不同分为PSP组(54例)和SP组(46例)。PSP组患者采用PSP治疗,SP组患者采用SP治疗。比较两组患者手术指标及并发症发生情况、复发情况。结果PSP组的手术时间(92.4±7.8)min、切口长度(8.20±1.25)cm以及术后住院时间(5.87±0.66)d均显著短于SP组的(128.8±9.6)min、(13.87±2.14)cm、(7.47±0.85)d,术中出血量(50.4±5.6)ml、解剖面神经支数(2.27±0.25)支均显著少于SP组的(82.4±10.4)ml、(3.01±0.34)支,差异均具有统计学意义(P<0.05)。PSP组患者的术后并发症发生率9.26%显著低于SP组的28.26%,差异具有统计学意义(P<0.05);术后随访12个月,两组患者的肿瘤复发率比较差异无统计学意义(P>0.05)。结论与SP相比,PSP对腮腺多形性腺瘤患者的创伤更小,有利于促进术后腮腺分泌功能的恢复,术后并发症少,且能切除肿瘤,不会增加肿瘤复发率,值得推广。Objective To investigate the effect of partial superficial parotidectomy(PSP)and superficial parotidectomy(SP)in the treatment of pleomorphic adenoma of the parotid gland.Methods A total of 100 patients with pleomorphic adenoma of the parotid gland were divided into PSP group(54 cases)and SP group(46 cases)according to different surgical methods.PSP group was treated with PSP,and SP group was treated with SP.The surgical indexes,complications and recurrence were compared between the two groups.Results In PSP group,the operative time was(92.4±7.8)min,the incision length was(8.20±1.25)cm,and the postoperative hospital stay was(5.87±0.66)d,which were significantly shorter than those of(128.8±9.6)min,(13.87±2.14)cm,and(7.47±0.85)d in SP group;the intraoperative blood loss of(50.4±5.6)ml and number of dissected facial nerve branches of(2.27±0.25)branches were significantly lower than those of(82.4±10.4)ml and(3.01±0.34)branches in SP group;the differences were statistically significant(P<0.05).The incidence of postoperative complications in PSP group was 9.26%,which was significantly lower than that of 28.26%in SP group,and the difference was statistically significant(P<0.05).After 12 months of follow-up,there was no statistically significant difference in tumor recurrence rate between the two groups(P>0.05).Conclusion Compared with SP,PSP has less trauma to patients with pleomorphic adenoma of the parotid gland,and is conducive to promoting the recovery of parotid secretion function after surgery with fewer postoperative complications.It can remove the tumor,and will not increase the recurrence rate of tumor,so it is worth promoting.
