血清CYFRA21-1、NSE、CEA、CA125肿瘤标志物联合检测肺癌的价值研究

摘　　要：目的分析血清细胞角蛋白19片段(CYFRA21-1)、神经元特异性烯醇化酶(NSE)、癌胚抗原(CEA)、糖类抗原125(CA125)肿瘤标志物联合检测肺癌的价值。方法选取80例肺癌患者作为研究组,另选同期82例良性肺结节患者作为对照组,两组患者均接受血清肿瘤标志物CYFRA21-1、NSE、CEA、CA125检测,比较两组患者血清CYFRA21-1、NSE、CEA、CA125水平及不同原发肿瘤-局部淋巴结-远处转移(TNM)分期肺癌患者血清CYFRA21-1、NSE、CEA、CA125水平,比较单独血清CYFRA21-1、NSE、CEA、CA125检测与四项肿瘤标志物联合检测肺癌的准确率。结果研究组血清CYFRA21-1(12.53±2.56)ng/ml、NSE(23.09±3.54)ng/ml、CEA(15.95±3.47)ng/ml、CA125(34.40±4.65)U/ml均高于对照组的(1.56±1.00)ng/ml、(7.20±2.43)ng/ml、(4.25±1.20)ng/ml、(17.85±3.78)U/ml,差异有统计学意义(P<0.05)。Ⅲ~Ⅳ期肺癌患者血清CYFRA21-1(15.65±3.00)ng/ml、NSE(26.03±3.56)ng/ml、CEA(19.02±2.77)ng/ml、CA125(38.46±3.79)U/ml均高于Ⅰ~Ⅱ期患者的(10.56±2.50)ng/ml、(21.23±3.47)ng/ml、(14.00±3.53)ng/ml、(32.64±4.02)U/ml,差异有统计学意义(P<0.05)。四项肿瘤标志物联合检测的准确率93.75%高于单独血清CYFRA21-1、NSE、CEA、CA125检测的56.25%、53.75%、47.50%、40.00%,差异均有统计学意义(P<0.05)。结论血清CYFRA21-1、NSE、CEA、CA125在肺癌诊断中具有一定的价值,且联合检测效果更为理想,为临床诊疗工作提供了重要的参考依据。Objective To analyze the value of combined detection of serum cytokeratin 19 fragment(CYFRA21-1),neuron specific enolase(NSE),carcinoembryonic antigen(CEA)and carbohydrate antigen 125(CA125)tumor markers in lung cancer.Methods 80 patients with lung cancer were selected as the study group,and another 82 patients with benign pulmonary nodules were selected as the control group.Both groups of patients were tested for serum tumor markers CYFRA21-1,NSE,CEA and CA125.Comparison was made on levels of serum CYFRA21-1,NSE,CEA and CA125 between the two groups,the levels of serum CYFRA21-1,NSE,CEA and CA125 among lung cancer patients with different tumor,nodal and metastatic(TNM)stages,as well as the accuracy of serum CYFRA21-1,NSE,CEA and CA125 alone and in combination for the detection of lung cancer.Results The study group had serum CYFRA21-1 of(12.53±2.56)ng/ml,NSE of(23.09±3.54)ng/ml,CEA of(15.95±3.47)ng/ml,and CA125 of(34.40±4.65)U/ml,which were higher than those of(1.56±1.00)ng/ml,(7.20±2.43)ng/ml,(4.25±1.20)ng/ml,and(17.85±3.78)U/ml in the control group,and the differences were statistically significant(P<0.05).Serum CYFRA21-1,NSE,CEA and CA125 were(15.65±3.00)ng/ml,(26.03±3.56)ng/ml,(19.02±2.77)ng/ml and(38.46±3.79)U/ml in patients with stageⅢ-Ⅳlung cancer,which were higher than those of(10.56±2.50)ng/ml,(21.23±3.47)ng/ml,(14.00±3.53)ng/ml,and(32.64±4.02)U/ml in patients with stageⅠ-Ⅱlung cancer,and the differences were statistically significant(P<0.05).The accuracy of 93.75%of the combined detection of four tumor markers was higher than that of 56.25%,53.75%,47.50%and 40.00%of the single detection of serum CYFRA21-1,NSE,CEA and CA125,and the differences were statistically significant(P<0.05).Conclusion Serum CYFRA21-1,NSE,CEA and CA125 have certain value in the diagnosis of lung cancer,and the effect of combined detection is more ideal,which provides an important reference for further clinical diagnosis and treatment.
