摘　　要：[目的]分析湖北省2017年恶性肿瘤发病、死亡以及疾病负担情况。[方法]收集湖北省肿瘤登记地区2017年恶性肿瘤发病、死亡资料,根据全国肿瘤登记中心制定的质量评价方法对数据进行审核,共有18个登记处的数据符合入选标准。采用描述性流行病学方法,通过计算发病/死亡率、年龄别发病/死亡率、中国人口标化率(中标率)、世界人口标化率(世标率)等指标对恶性肿瘤的发病和死亡情况进行综合分析,同时根据世界卫生组织提供的公式计算伤残调整寿命年(disability-adjusted life years,DALY)和DALY率来估计恶性肿瘤的疾病负担。[结果] 2017年湖北省肿瘤登记地区新发病例数54 063例,发病率为284.54/10万,中标率为183.12/10万,世标率为197.42/10万,0~74岁累积率为20.75%,35~64岁截缩率为304.68/10万。2017年湖北省肿瘤登记地区恶性肿瘤死亡数32 627例,死亡率为171.72/10万,中标率为102.88/10万,世标率为116.86/10万,0~74岁累积率为11.85%,35~64岁截缩率为131.95/10万。全省肿瘤登记地区恶性肿瘤发病和死亡顺位中肺癌均居首位。2017年湖北省肿瘤登记地区恶性肿瘤的早死寿命损失年(years of life lost,YLL)总体为416 258人年,YLL率为21.9/千人。伤残寿命损失年(years lived with disability,YLD)总体为101 652人年,YLD率为5.4/千人。DALY总体为517 910人年,DALY率为27.3/千人。[结论]恶性肿瘤年龄别发病率和死亡率均在80~84岁年龄组达到峰值,中老年人群依旧是癌症防治的重点关注对象,同时也要针对重点癌种展开筛查和早诊早治,以降低疾病负担,提高生存质量。[Purpose]To analyze the incidence,mortality and disease burden of malignant tumors in cancer registration areas of Hubei Province in 2017.[Methods]The incidence and mortality data of malignant tumors in cancer registration areas of Hubei Province in 2017 were collected.The data were reviewed according to the quality evaluation method formulated by the National Cancer Registration Center,and 18 registries met the inclusion criteria.Using descriptive epi-demiological methods,the incidence/mortality rate,age-specific incidence/mortality rate,age-standardized rate by Chinese standard population(ASR China),age-standardized rate by world population(ASR world)and other indicators were calculated.At the same time,the disability-ad-justed life years(DALY)and DALY rate were calculated according to the formula provided by the World Health Organization to estimate the disease burden of malignant tumors.[Results]In 2017,there were 54063 new cancer cases in cancer registration areas of Hubei Province,the crude incidence rate was 284.54/105,the ASR China was 183.12/105,the ASR world was 197.42/105,the cumulative rate(0~74 years old)was 20.75%,and the truncation rate(35~64 years old)was 304.68/105.In 2017,there were 32627 deaths from malignant tumors in cancer registration areas of Hubei Province.The crude mortality rate was 171.72/105,the ASR China was 102.88/105,the ASR world was 116.86/105,the cumulative rate(0~74 years old)was 11.85%,and the truncation rate(35~64 years old)was 131.95/105.Lung cancer ranked first in the incidence and mortality of malignant tumors.In 2017,the YLL of malignant tumors in cancer registration areas of Hubei Province was 416258 person-years,and the YLL rate was 21.9 per 1000.The overall YLD was 101652 person-years,and the YLD rate was 5.4 per 1000.The overall DALY was 517910 per-son-years,and the DALY rate was 27.3 per 1000.[Conclusion]The age-specific incidence and mortality of malignant tumors peaked at the age group of 80~84 years.The middle-aged and el-derly people are still the focus of cancer
