摘　　要：目的:探讨胆管癌患者血清长链非编码RNA母系表达基因3(lncRNA MEG3)表达及其与预后的关系。方法:选取胆管癌患者92例(胆管癌组)和体检健康者92例(对照组)为研究对象。术后随访36个月,根据预后情况分为预后不良组(44例,患者死亡)和预后良好组(48例,患者存活)。采用荧光定量PCR检测血清lncRNA MEG3表达水平。比较对照组与胆管癌组血清lncRNA MEG3表达水平。比较预后良好组和预后不良组临床资料、病理特征及血清lncRNA MEG3表达水平。采用Spearman法分析胆管癌患者血清lncRNA MEG3表达水平与肿瘤分化程度、TNM分期、淋巴结转移、血管浸润的相关性。采用多因素Logistic回归分析胆管癌患者预后不良的影响因素。采用受试者工作特征(ROC)曲线评价血清lncRNA MEG3对胆管癌患者预后不良的预测价值。结果:胆管癌组血清lncRNA MEG3表达水平低于对照组(P<0.05)。预后不良组肿瘤低分化、TNM分期Ⅲ期、有淋巴结转移、有血管浸润比例高于预后良好组,血清lncRNA MEG3表达水平低于预后良好组(均P<0.05)。胆管癌患者血清lncRNA MEG3表达水平与肿瘤分化程度呈正相关,与TNM分期、淋巴结转移、血管浸润呈负相关(均P<0.05)。肿瘤低分化、TNM分期Ⅲ期、有淋巴结转移、血清lncRNA MEG3低表达是胆管癌患者预后不良的独立危险因素(均P<0.05)。血清lncRNA MEG3预测胆管癌患者预后不良的曲线下面积为0.820,截断值为0.620,敏感度为87.10%,特异度为70.80%。结论:胆管癌患者血清lncRNA MEG3呈低表达,与患者预后密切相关,对预测胆管癌患者预后不良有较高预测价值。Objective:To investigate the expression of serum long non-coding RNA maternally expressed gene 3(lncRNA MEG3)in patients with cholangiocarcinoma and its correlation with prognosis.Methods:A total of 92 patients with cholangiocarcinoma(cholangiocarcinoma group)and 92 healthy subjects(control group)were selected.The patients with cholangiocarcinoma were followed up for 36 months after operation and divided into poor prognosis group(44 cases,died)and good prognosis group(48 cases,survived)according to the prognosis.Serum lncRNA MEG3 expression was detected by fluorescence quantitative PCR.The serum lncRNA MEG3 expression level of control group and cholangiocarcinoma group were compared.The clinical data,pathological features and serum lncRNA MEG3 expression level of good prognosis group and poor prognosis group were compared.Spearman assay was used to analyze the correlation between serum lncRNA MEG3 expression level and tumor differentiation,TNM stage,lymph node metastasis and vascular infiltration in patients with cholangiocarcinoma.Multivariate Logistic regression was used to analyze the influencing factors of poor prognosis in patients with cholangiocarcinoma.ROC curve was used to evaluate the predictive value of serum lncRNA MEG3 for poor prognosis of patients with cholangiocarcinoma.Results:The serum lncRNA MEG3 expression level of cholangiocarcinoma group was significantly lower than that of control group(P<0.05).The proportion of tumor with low differentiation,TNM stageⅢ,lymph node metastasis and vascular infiltration in poor prognosis group was significantly higher than that in good prognosis group,and the serum lncRNA MEG3 expression level was significantly lower than that in good prognosis group(all P<0.05).Serum lncRNA MEG3 expression level in patients with cholangiocarcinoma was positively correlated with the degree of tumor differentiation,and negatively correlated with TNM staging,lymph node metastasis,and vascular infiltration(all P<0.05).Low tumor differentiation,TNM stageⅢ,lymph node metastasis
