摘 要:目的探究龋齿合并卟啉单胞菌(Pg)感染患儿外周血核因子κB(NF-κB)信号通路表达。方法选取2019年6月-2021年6月于南京医科大学附属儿童医院就诊的100例龋齿患儿为研究组,根据患儿牙龈Pg感染情况将研究组患儿分为感染组与未感染组,另选取同期接受体检的85名健康体检儿童为对照组。采用酶联免疫吸附法检测外周血NF-κB、干扰素-γ(IFN-γ)、白细胞介素-1β(IL-1β)和肿瘤坏死因子-α(TNF-α)水平;实时荧光定量聚合酶链反应法检测NF-κBp65、核因子κB激活剂(Act1)和NF-κB抑制蛋白α(IκBα)的mRNA的表达水平;多元线性回归分析法分析NF-κB信号通路相关细胞因子水平与Pg感染的相关性,受试者工作特征(ROC)曲线评估其对龋齿患儿Pg感染的诊断价值。结果研究组Pg感染阳性率、Pg浓度均高于对照组(P<0.05);感染组外周血NF-κB、IL-1β、TNF-α、NF-κBp65-mRNA、Act1-mRNA和IκBα-mRNA均高于未感染组,IFN-γ低于未感染组(P<0.05);NF-κB、IFN-γ、IL-1β和TNF-α与龋齿患儿Pg感染存在相关性(P<0.05);ROC曲线分析显示,NF-κB、IFN-γ、IL-1β和TNF-α诊断龋齿患儿Pg感染的曲线下面积(AUC)分别为0.819、0.803、0.798和0.748(P<0.05)。结论Pg感染会使NF-κB信号通路过度活化,从而影响相关细胞因子的表达;NF-κB信号通路相关细胞因子检测对龋齿患儿Pg感染的诊断具有指导意义。OBJECTIVE To investigate the expression of peripheral blood nuclear factor-κB(NF-κB)signaling pathway in children with dental caries and Porphyromonas gingivalis(Pg)infection.METHODS A total of 100 children with dental caries treated in the Children's Hospital of Nanjing Medical University between Jun.2019 and Jun.2021 were selected as the study group,and the children in the study group were divided into infection group and non-infection group according to the presence or absence of Pg infection.Meanwhile,85 healthy children who underwent physical examination during the same period were selected as the control group.The levels of peripheral blood NF-κB,interferon-γ(IFN-γ),interleukin-1β(IL-1β)and tumor necrosis factor-α(TNF-α)were detected by enzyme-linked immunosorbent assay.The mRNA levels of NF-κBp65,factorκB activator(Act1)and NF-κB inhibitory proteinα(IκBα)were detected by real-time fluorescence quantification polymerase chain reaction.The correlation between the levels of NF-κB signaling pathway-related cytokines and Pg infection was analyzed by multiple linear regression analysis method,and the receiver operating characteristic(ROC)curve was used to assess their diagnostic value for Pg infection in children with dental caries.RESULTS The positive rate of Pg infection and concentration of Pg in the study group were higher than those in the control group(P<0.05).Peripheral blood NF-κB,IL-1β,TNF-α,NF-κBp65-mRNA,Act1-mRNA and IκBα-mRNA in the infection group were higher than those in the non-infection group,while IFN-γwas lower than that in the non-infection group(P<0.05).NF-κB,IFN-γ,IL-1βand TNF-αwere related to Pg infection in children with dental caries(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of NF-κB,IFN-γ,IL-1βand TNF-αfor the diagnosis of Pg infection in children with dental caries was 0.819,0.803,0.798 and 0.748,respectively(P<0.05).CONCLUSION Pg infection caused excessive activation of the NF-κB signaling pathway,thereby affecting the expressi
