摘  要:目的基于颈椎骨龄定量分期法,比较青少年骨性Ⅰ类和Ⅱ类错牙合畸形患者在不同颈椎骨龄阶段舌骨位置的差异。方法从门诊选择8-18岁在厦门医学院附属口腔医院正畸科就诊的错牙合畸形患者360例,男女各180例,男性平均13.5岁,女性平均13.2岁,其中男性骨性Ⅰ类(90例)和男性骨性Ⅱ类(90例),女性骨性Ⅰ类(90例)和女性骨性Ⅱ类(90例),根据颈椎骨龄定量分期法分别将男性和女性的骨性Ⅰ类及骨性Ⅱ类分为高峰前期组(30例)、高峰期组(30例)和高峰后期组(30例),所有患者在正畸治疗前均拍摄CBCT,以dicom格式导入Dolphin软件中截取正中矢状面图像,并对舌骨在垂直向及水平向位置进行定点,测量相关的角度及距离,应用SPSS 21.0统计学软件进行统计分析。结果在颈椎骨龄高峰前期,男性骨性Ⅰ类及Ⅱ类错牙合畸形患者∠CSN(第三颈椎与前颅底交角)及∠HSN(舌骨与前颅底交角)的差异有统计学意义,女性S-H(蝶鞍点与舌骨之间的距离)差异有统计学意义;在颈椎骨龄发育高峰期,男性和女性∠CSH(颈椎、蝶鞍点和舌骨的交角)及H-C3(舌骨与第三颈椎的距离)差异均有统计学意义,另外,男性S-C3(蝶鞍点与第三颈椎的距离)及女性H-RGN(舌骨至下颌颏部后下缘的距离)差异也有统计学意义;在颈椎骨龄高峰后期,男性所有测量项目差异均无统计学意义,但男性H-C3及H-RGN实际测量值骨性Ⅱ类均小于骨性Ⅰ类,而女性∠CHS、H-C3及H-RGN差异有统计学意义。结论从颈椎骨龄的高峰前期至高峰后期,男性和女性骨性Ⅰ类与Ⅱ类的舌骨位置均向前下方向移位;在高峰后期,男性和女性骨性Ⅱ类的舌骨位置均比骨性Ⅰ类水平向更靠后,而垂直向两者没有区别。Objective To compare the position of hyoid bone between skeletal classⅠand classⅡmalocclusion based on quantitative cervical vertebra maturation(QCVM).Methods The total of 360 patients aged 8~18 years were taken from the orthodontic department and divided into tow groups,including 180 males with the average age of 13.5 and 180 females with the average age of 13.2.In the first,these two groups were respectively divided into 90 skeletal classⅠmalocclusion subgroup and 90 skeletal classⅡmalocclusion subgroup according to the ANB angle.Secondly,each subgroup was divided into pre-peak period group,peak period group and post-peak period group with 30 patients respectively based on quantitative cervical vertebra maturation(QCVM).CBCT was taken before orthodontic treatment and then imported into the Dolphin software.The median sagittal image was selected to measure the angle and distance of the hyoid bone in vertical and horizontal directions.All data were analyzed by SPSS 21.0 software.Results In the pre-peak period,the angle of CSN and HSN were signifificantly difffferent for males and the distance of S-H was also signifificantly difffferent for females between skeletal classⅠand classⅡmalocclusion.In the peak period,the CSH and H-C3 were signifificantly difffferent for both males and females.The S-C3 for males and H-RGN for females were also signifificantly difffferent.In the post-peak period,none of the measurement has the signifificant difffference for males,but the distance of H-C3 and H-RGN were more short in skeletal classⅡmalocclusion than classⅠ.The CHS,H-C3 and H-RGN were signifificantly difffferent for females in the post-peak period.Conclusion From the pre-peak period to the post-peak period of quantitative cervical vertebra maturation(QCVM),the growth tendency of hyoid bone directed forward and downward both skeletal classⅠand classⅡmalocclusion for males and females.In addition,the position of hyoid bone was more backward in skeletal classⅡmalocclusion than classⅠmalocclusion for bo
