摘 要:目的 观察苗药五藤膏外敷对膝关节骨关节炎(KOA)兔关节软骨中糖原合酶激酶-3β(GSK-3β)、Notch4表达的影响,探讨苗药五藤膏治疗KOA的可能作用机制。方法 将同批次健康的72只新西兰大白兔随机分为空白组、模型组、巴布膏组及苗药五藤膏低、中、高剂量组,每组12只。除空白组外,其余组均采用木瓜蛋白酶膝关节内注射方法建立兔KOA模型。实验第5周开始,巴布膏组给予巴布膏外敷右膝关节,苗药五藤膏低、中、高剂量组分别给予约1 g、2 g、4 g的苗药五藤膏外敷右膝关节,空白组和模型组采用无菌纱布包扎右膝关节,均1次/d,每次固定4 h,干预4周。HE染色观察兔软骨组织病理形态并进行病理学Mankin评分,Western blot法检测关节软骨组织中GSK-3β、Notch4蛋白表达情况。结果 模型组兔软骨Mankin评分及软骨组织中GSK-3β、Notch4蛋白相对表达量均明显高于空白组(P均<0.05),各药物组Mankin评分及软骨组织中GSK-3β、Notch4蛋白相对表达量均明显低于模型组(P均<0.05);苗药五藤膏高剂量组Mankin评分明显低于其他药物组(P均<0.05),软骨组织中Notch4蛋白相对表达量明显低于苗药五藤膏低、中剂量组(P均<0.05)。结论 苗药五藤膏能延缓KOA病情进展,机制可能与抑制KOA兔软骨GSK-3β、Notch4蛋白表达有关。Objective It is to observe the effect of external application of the Miao nationality herbal medicine Wuteng ointment on the expression of glycogen synthase kinase-3β(GSK-3β)and Notch4 in articular cartilage of rabbits with knee osteoarthritis(KOA),and to investigate the possible mechanism of this medicine in the treatment of KOA.Methods Seventy-two healthy New Zealand white rabbits in the same batch were randomly divided into blank group,model group,Babu ointment group and low,medium and high dose groups of Wuteng ointment,with 12 rabbits in each group.All the rabbits in each group except for the blank group were given papain by internal knee injection to establish rabbit KOA models.Starting from the 5th week of the experiment,the Babu ointment group was treated with external application of Babu ointment on the right knee joint,the low,medium and high dose groups of Wuteng ointment were respectively treated with external application of about 1 g,2 g and 4 g of Wuteng ointment on the right knee joint,and the right knee joint was bandaged with sterile gauze in the blank group and model group,all of which were fixed for 4 h,once daily,and treated for 4 weeks.The pathological morphology of rabbit cartilage tissues was observed by HE staining,and was evaluated by pathological Mankin scoring,the expressions of GSK-3βand Notch4 proteins in articular cartilage tissues were detected by Western blot.Results The Mankin score of rabbit cartilage and the relative expressions of GSK-3βand Notch4 proteins in cartilage tissues of the model group were significantly higher than those of the blank group(all P<0.05),and the Mankin score and the relative expressions of GSK-3βand Notch4 proteins in cartilage tissues of each treatment group were significantly lower than those of the model group(all P<0.05);the Mankin score of the high-dose group of Wuteng ointment was significantly lower than that of the other treatment groups(all P<0.05),and the relative expression of Notch4 proteins in cartilage tissues was significantly lower
