小儿并趾分趾后M-V嵌合皮瓣重建趾蹼的临床研究

摘　　要：目的研究M-V嵌合皮瓣并趾分趾后重建趾蹼的临床疗效.方法我科自2015年3月至2021年1月采用M-V嵌合皮瓣重建并趾趾蹼患儿12例.不完全性并趾11足,完全性并趾1足.由患儿家属对术后重建指蹼满意度进行评定,采用温哥华评分评价趾蹼瘢痕,根据趾蹼坡度、深度及足趾外展度评估重建效果.结果本组皮瓣全部存活,术后随访12~24个月,平均15个月,重建的趾蹼外观、质地、皮瓣颜色和健侧趾蹼近似,患儿家属对外观均满意.趾蹼瘢痕采用温哥华评分:色泽(0.7±0.3)分,血管分布0分,柔软度(1.1±0.6)分,厚度(1.3±0.7)分.重建趾蹼坡度、深度、足趾外展度和健侧比较,差异无统计学意义.结论M-V嵌合皮瓣对趾蹼区皮肤利用率高,缩短了手术及住院时间,外形美观,是并趾分趾后重建趾蹼的理想方法之一.Objective To investigate the clinical efficacy of M-V chimeric flap for reconstruction of toe web in children following toe syndactyly surgery.Methods From March 2015 to January 2021,12 children with toe syndactyly were performed reconstruction of toe web used by M-V chimeric flaps.There were 11 incomplete syndactyly and 1 complete syndactyly.The satisfaction of postoperative reconstruction of the web was evaluated by the family members of the patient.The Vancouver score was used to evaluate web scars,and the reconstruction efficacy was evaluated based on web slope,depth,and toe abduction.Results All the flaps survived,and the postoperative follow-up was 12 to 24 months,with an average of 15 months.The appearance,texture,and color of the reconstructed toe web were similar to those of the healthy side,and the patient's family members were satisfied with the appearance.The Vancouver score used for toe web scar evaluation was(0.7±0.3)points in color,0 in vascular distribution,(1.1±0.6)points in softness,and(1.3±0.7)points in thickness.There was no statistically significant difference in slope,depth,toe abduction of reconstructed toe web between affected side and healthy side.Conclusion The M-V chimeric flap has a high utilization rate for the skin of the toe web area,shortens the surgical and hospitalization time,and has a beautiful appearance.It is one of the ideal methods for reconstructing the toe web after toe syndactyly surgery.
