优化防絮凝祛泡剂在无痛胃镜检查中的可视度研究

摘  要:目的研究经定比定量的优化药物参数合成的防絮凝祛泡剂新配方在无痛胃镜检查中应用效果。方法选取解放军总医院第二医学中心2021年5月~2022年11月接受无痛胃镜检查1200例患者,随机数表法分成优化新配方的观察组(西甲硅油乳剂400 mg∕人、链霉蛋白酶20000 U∕人、碳酸氢钠散1.6 g∕人、木糖醇1.0 g∕人)和原配方的对照组(西甲硅油乳剂400 mg∕人、链霉蛋白酶20000 U∕人、碳酸氢钠散1.0 g∕人),各600例。对比分析两组患者胃内黏膜可视度及白色絮凝情况、瘤变检出率;以及两组祛泡剂配置后0、1、2和3 h祛泡力及抑泡力变化。结果观察组检查中食管、胃内无絮凝且黏膜下可视度评级优势与对照组比较,差异有统计学意义(P<0.05)。两组食管、胃黏膜病灶检出率差异有统计学意义(P<0.05)。两组配置后0、1、2和3 h的pH值及黏膜下可视度评级差异有统计学意义(P<0.05),观察组A级可视度百分率明显高于对照组,且无C、D级。提示观察组祛泡力及抑泡力强、稳定性好且口感舒适度优良。结论新配方胃镜检查中食管及胃内无絮凝,具有优质的黏膜下可视度;配置后3 h内pH值及祛泡祛粘液效力未受明显干扰,表现出良好稳定性,且有口感优势,提高了食管、胃黏膜病灶的检出。Objective To study the application effect of the optimized antifoam new formula in painless gastroscopy.Methods 1200 patients who underwent painless gastroscopy in our hospital from May 2021 to November 2022 were randomly divided into new formula as observation group(silicone oil emulsion 400 mg∕person,streptomycin 20000 U∕person,sodium bicarbonate powder 1.6 g∕person,xylitol 1.0 g∕person)and original formula as control group(silicone oil emulsion 400 mg∕person,streptomycin 20000 U∕person,sodium bicarbonate powder 1.0 g∕person).The visibility of gastric mucosa,white sediment and detection rate of tumor in observation group and control group were compared and analyzed.The changes of defoaming ability and antifoaming ability of the two groups at 0,1,2 and 3 h after the preparation of defoaming agent were compared.Results There was no sediment in the esophagus and stomach of patients in observation group,and the submucosal visibility rating was higher than that in control group.There was a significant difference between the two groups(P<0.05).There was a difference in the detection rate of esophageal and gastric mucosal lesions between observation group and control group(P<0.05).There were differences in pH values and submucosal visibility ratings between observation group and control group at 0,1,2 and 3 h after configuration(P<0.05).The percentage of grade A visibility in observation group was significantly higher than that in control group,and there was no grade C or D.It is suggested that observation group has strong defoaming and antifoaming power,good stability and good taste comfort.Conclusion There is no flocculation in the esophagus and stomach in the new formula gastroscopy,and it has high-quality submucosal visibility.The pH value and the effect of removing bubbles and mucus were not significantly disturbed within 3 h after the preparation,showing good stability and taste advantages,which improved the detection of esophageal and gastric mucosal lesions.
