前牙残根正畸牵引加牙冠延长在前牙牙体缺损修复中的应用效果观察

摘　　要：目的:探究应用正畸牵引术结合牙冠延长术的方法在前牙牙体缺损修复中的效果。方法:选取2019年3月至2022年3月本院收治的92例前牙牙体缺损患者作为研究对象,并按随机分组法分成对照组和观察组,各46例。对照组采用单独牙冠延长术治疗,观察组在对照组的基础上结合正畸牵引术。治疗2m后进行随访比较两组的前牙修复有效率、术后合并症的发生情况和患者满意度评分的变化,并以牙齿松动度(Tooth mobility,TM)、菌斑指数(Plaque index,PLI)、牙周袋深度(Pocket depth,PD)、牙龈指数(Gingival index,GI)评估患者牙周情况。结果:治疗后,观察组的前牙修复有效率明显高于对照组(P<0.05),术后合并症发生率明显低于对照组(P<0.05);两组TM、PLI、PD、GI水平均明显降低(P<0.05),且观察组明显低于对照组(P<0.05);观察组牙齿的美观性、咀嚼功能、言语功能、稳固性的满意度评分均明显高于对照组(P<0.05)。结论:相比于单纯应用牙冠延长术,正畸牵引术联合牙冠延长术的方案在前牙牙体缺损的患者中疗效更加显著,能改善牙周指数水平,提高修复效果和满意度,减少术后合并症的发生。Objective:To explore theapplication effect of orthodontic traction combined with crown extension in the repair of anterior tooth defects.Methods:A total of 92 patients with anterior tooth defects admitted to the hospitalwere enrolled as the research objectsbetween March 2019 and March 2022.According to random grouping method,they were divided into control group(46 cases)and observation group(46 cases).The control group was treated with crown extension,while observation group was additionally treated with orthodontic traction.After 2 months of treatment,all were followed up to compare changes in response rate of anterior tooth repair,occurrence of postoperative complications and scores of patients satisfaction.The periodontal conditions were evaluated by tooth mobility(TM),plaque index(PLI),pocket depth(PD)and gingival index(GI).Results:After treatment,response rate of anterior teeth repairin observation group wassignificantlyhigher than that in control group(P<0.05),and incidence of postoperative complications was significantly lower than that in control group(P<0.05).After treatment,levels of TM,PLI,PD andGIin both groups were significantly decreased(P<0.05),which were significantly lowerin observation group than control group(P<0.05).The scores of patients satisfaction with teeth aesthetics,chewing function,speech function and dental stability in observation group were significantly higher than those in control group(P<0.05).Conclusion:Compared with crown extensionalone,curative effect of orthodontic traction combined with crown extensionis more significant onpatients with anterior tooth defects,which can improveperiodontal indexes,repair effect and satisfaction,and reduce postoperative complications.
