早期拔管联合局部持续负压引流在乳腺癌改良根治术中的应用效果分析

摘　　要：目的探讨早期拔管联合局部持续负压引流在乳腺癌改良根治术中的应用效果。方法选取2018年1月至2021年12月收治的42例行乳腺癌改良根治术的患者为研究对象,随机分为两组,常规拔管组21例,术后连续2 d引流量低于10 mL时拔除引流管,早期拔管组21例,术后连续2 d引流量低于25 mL时拔除引流管。出现皮下积液时采取局部穿刺置管持续负压引流处理。观察两组术后皮下积液的发生率、平均住院日、平均住院费用、平均置管天数。结果两组皮下积液发生率、平均住院日、平均住院费用比较差异无统计学意义(P>0.05)。早期拔管组平均置管天数短于常规拔管组,差异有统计学意义(P<0.05)。结论早期拔除负压引流管联合局部持续负压引流,可以明显减少患者引流管留置天数,改善术后生活质量,有利于创面愈合和患者下一步的综合治疗。Objective To investigate the effect of early drain removal combined with local continuous low vacuum drainage in modified radical mastectomy.Methods Forty-two patients with breast cancer who received modified radical mastectomy from January 2018 to December 2021 were selected as the research object,and were randomly divided into the conventional drain removal group of 21 cases.The drainage tube was removed when the drainage volume was less than 10 mL for two consecutive days after surgery.In the early drain removal group of 21 cases,the drainage tube was removed when the drainage volume was less than 25 mL for two consecutive days.When seroma formation,local puncture and continuous low vacuum drainage should be performed.Observe the incidence of postoperative Seroma formation,average hospital stay,average hospital expenses,average number of catheterization days,and incision infections in both groups.Result There was no statistically significant difference in the incidence of seroma formation,average hospital expenses,and average hospital stay between the two groups(P>0.05).The average catheterization day in the conventional drain removal group was higher than that in the early drain removal group,and the difference was statistically significant(P<0.05).Conclusion Early drain removal combined with local continuous negative pressure drainage can significantly shorten the duration of tube placement,improve patients'quality of life,and be beneficial for wound healing and comprehensive treatment in the next step.
