摘　　要：目的研究尼可地尔(Nico)对消化道肿瘤根治术患者心肌损伤的影响。方法将消化道肿瘤根治术患者30例分全身麻醉对照组(C组,n=15)与全身麻醉尼可地尔组(N组,n=15)。C组常规实施全身麻醉。N组在全身麻醉对照组的基础上,术前30 min以80μg/(kg·h)速率静脉泵注Nico至手术结束停止。观察两组术后24 h丙二醛(MDA)和白细胞介素-6(IL-6)、心肌肌钙蛋白T(cTnT)差异性变化。结果两组术前MDA、IL-6、cTnT水平比较无显著差异。与T_(0)比较,T_(1)时C组MDA、IL-6、cTnT水平显著增高(P<0.05),N组无显著变化(P>0.05);与C组比较,N组T_(0)时MDA、IL-6、cTnT水平无显著变化(P>0.05),T_(1)时显著降低(P<0.05)。结论Nico通过抑制术后MDA、IL-6、cTnT升高,减轻围手术期患者心肌损伤,是值得推广的术前用药。Objective To study the effect of nicorandil on myocardial injury in patients undergoing radical resection of gastrointestinal tumors.Methods The 30 patients undergoing radical resection of gastrointestinal tumors were divided into general anesthesia control group(group C,n=15)and general anesthesia nicorandil group(N group,n=15).Group C routinely performs general anesthesia.In the N group,Nico was pumped intravenously at a rate of 80μg/(kg·h)30 minutes before surgery on the basis of the general anesthesia control group until the end of the operation.The differential changes of malondialdehyde(MDA),interleukin-6(IL-6)and cardiac troponin T(cTnT)at 24 h after surgery were observed between the two groups.Results There were no significant differences in preoperative MDA,IL-6 and cTnT levels between the two groups.Compared with T_(0),the levels of MDA,IL-6 and cTnT in group C were significantly increased at T_(1)(P<0.05),but there was no significant change in group N(P>0.05).Compared with group C,there was no significant change in MDA,IL-6 and cTnT levels in T_(0)(P>0.05)and significantly decreased at T_(1)(P<0.05).Conclusion Nico can reduce myocardial injury in perioperative patients by inhibiting the increase of MDA,IL-6 and cTnT.It is worthy of popularization.
