摘　　要：目的:研究鼻外入路泪囊鼻腔吻合术(external dacryocystorhinostomy,Ex-DCR)与鼻内镜下泪囊鼻腔造孔术(in endoscopic dacryocystorhinostomy,IECR)治疗复发性泪囊炎患者的效果差异。方法:选取2020年1月─2022年1月绵阳市中心医院收治的80例复发性泪囊炎患者为研究对象,采用随机数表法将其分为试验组(n=40)和对照组(n=40)。对照组行Ex-DCR治疗,试验组行IECR治疗。比较两组手术治疗效果、手术相关指标、术后12个月内溢泪症状评分。结果:两组治疗有效率比较,差异无统计学意义(P>0.05)。两组手术持续时间、术中出血量、总住院时间比较,差异无统计学意义(P>0.05);试验组总治疗费用高于对照组,并发症发生率低于对照组,差异有统计学意义(P<0.05)。两组术后3个月、6个月溢泪症状评分比较,差异无统计学意义(P>0.05);试验组术后12个月溢泪症状评分低于对照组,差异有统计学意义(P<0.05)。结论:Ex-DCR与IECR对复发性泪囊炎患者均有良好的短期治疗效果,但IECR对复发性泪囊炎患者的安全性更高,溢泪症状改善效果的维持时间更长,手术费用也更高。Objective:To study the difference in efficacy between external dacryocystorhinostomy(Ex-DCR)and in endoscopic dacryocystorhinostomy(IECR)in the treatment of recurrent dacryocystitis patients.Method:A total of 80 patients with recurrent dacryocystitis treated in Mianyang Central Hospital from January 2020 to January 2022 were selected as the study objects,they were divided into experimental group(n=40)and control group(n=40)by random number table method.The control group was treated with Ex-DCR and the experimental group was treated with IECR.The results of surgical treatment,surgical related indexes and the scores of lacrimal discharge symptoms 12 months after surgery were compared between the two groups.Result:There was no significant difference in treatment effectiveness between the two groups(P>0.05).There were no significant differences in operation duration,intraoperative blood loss and total hospital stay between the two groups(P>0.05).The total treatment cost of the experimental group was higher than that of the control group,and the complication rate was lower than that of the control group,the differences were statistically significant(P<0.05).There were no significant differences in tear discharge symptoms scores between the two groups at 3 months and 6 months after operation(P>0.05).The tear discharge symptoms score of experimental group was lower than that of control group at 12 months after operation,the difference was statistically significant(P<0.05).Conclusion:Both Ex-DCR and IECR have good short term treatment effect on patients with recurrent dacryocystitis,but IECR has higher safety for patients with recurrent dacryocystitis,longer maintenance time for improvement of tear discharge symptoms,and higher surgical costs.
