双源CT小肠造影联合X线钡餐造影对克罗恩病的诊断价值

摘　　要：目的:探究双源CT小肠造影(CT enterography,CTE)联合X线钡餐造影对克罗恩病(Crohn disease,CD)的诊断价值。方法:回顾性分析2021年1月-2022年12月厦门大学附属第一医院收治的96例炎症性肠病患者的临床资料。根据结肠镜检查结果及临床表现等将其分为CD(n=61)、溃疡性结肠炎(n=35)。所有患者均进行双源CTE及X线钡餐造影、结肠镜检查。以结肠镜检查结果及临床表现等为金标准,分析双源CTE、X线钡餐造影及联合检查的诊断结果及其与金标准的一致性。比较双源CTE、X线钡餐造影及联合检查对CD的诊断价值。比较双源CTE、X线钡餐造影对CD特征性病变的检出情况。分析CD患者的影像学图片。结果:联合检查与金标准诊断一致性(Kappa=0.738,P<0.001)大于双源CTE(Kappa=0.661,P<0.001)、X线钡餐造影(Kappa=0.570,P<0.001)。联合检查敏感度大于双源CTE、X线钡餐造影,差异有统计学意义(P<0.05)。双源CTE对肠壁增厚、强化增加、蜂窝织炎、炎性包块、腹腔脓肿的检出率均高于X线钡餐造影,差异有统计学意义(P<0.05)。结论:双源CTE联合X线钡餐造影对CD的诊断价值较高,且双源CTE对CD特征性病变的检出率高于X线钡餐造影。Objective:To explore the diagnostic value of dual-source CT enterography(CTE)combined with X-ray barium meal radiography on Crohn disease(CD).Method:The clinical data 96 patients with inflammatory bowel disease admitted to the First Affiliated Hospital of Xiamen University from January 2021 to December 2022 were retrospectively analyzed.According to the results of colonoscopy and clinical manifestations etc,they were divided into CD(n=61)and ulcerative colitis(n=35).All patients underwent dual-source CTE,X-ray barium meal radiography and colonoscopy examination.With colonoscopy results and clinical manifestations etc as the gold standard,the diagnostic results of double-source CTE,X-ray barium meal radiography and combined examination and their consistency with the gold standard were analyzed.The diagnostic value of dual-source CTE,X-ray barium meal radiography and combined examination on CD was compared.The detection condition of CD characteristic lesions by dual-source CTE and X-ray barium meal radiography was compared.The imaging images of CD patients were analyzed.Result:The diagnosis consistency of combined examination and gold standard(Kappa=0.738,P<0.001)was greater than that of dual-source CTE(Kappa=0.661,P<0.001)and X-ray barium meal radiography(Kappa=0.570,P<0.001).The sensitivity of combined examination was higher than that of dual-source CTE and X-ray barium meal radiography,and the difference was statistically significant(P<0.05).The detection rates of dual-source CTE for intestinal wall thickening,enhancement,cellulitis,inflammatory mass and abdominal abscess were higher than those of X-ray barium meal radiography,and the differences were statistically significant(P<0.05).Conclusion:Dual-source CTE combined with X-ray barium meal radiography has higher diagnostic value for CD,and the detection rate of dual-source CTE for CD characteristic lesions is higher than that of X-ray barium meal radiography.
