摘  要:目的:对比分析显微手术夹闭与血管内介入治疗颅内动脉瘤的效果及对血管内皮功能的影响。方法:选择2020年7月-2022年5月于福建医科大学附属龙岩第一医院神经外科住院的83例颅内动脉瘤患者作为研究对象。按治疗方法的不同将其分为A组(n=42)与B组(n=41),A组采用显微手术夹闭治疗,B组采用血管内介入治疗,对比两组疗效、手术相关指标、血管内皮功能及术后并发症。结果:B组总有效率为85.37%,明显高于A组的61.90%,差异有统计学意义(P<0.05);B组术中出血量少于A组,手术时间及住院时间均短于A组,差异有统计学意义(P<0.05);术后3个月,两组血管假性血友病因子(vWF)、内皮素-(ET-1)水平均下降,一氧化氮(NO)水平升高,且B组vWF、ET-1水平低于A组,NO水平高于A组,差异有统计学意义(P<0.05);B组并发症发生率为7.32%,显著低于A组的23.81%,差异有统计学意义(P<0.05)。结论:与显微手术夹闭相比,血管内介入手术对颅内动脉瘤患者的疗效更好,对血管内皮损伤更小。Objective:To comparatively analyze the effect of microsurgical clipping and endovascular intervention in the treatment of intracranial aneurysm and its influence on vascular endothelial function.Method:A total of 83 patients with intracranial aneurysm who hospitalized in the Department of Neurosurgery,Longyan First Hospital Affiliated to Fujian Medical University from July 2020 to May 2022 were selected as the research objects.They were divided into group A(n=42)and group B(n=41)according to different treatment methods,group A was given microsurgical clipping,group B was given endovascular intervention,the efficacy,surgical related indicators,vascular endothelial function and postoperative complications were compared between two groups.Result:The total effective rate of group B was 85.37%,which was significantly higher than 61.90%of group A,the difference was statistically significant(P<0.05).The intraoperative blood loss in group B was less than that in group A,and the operation time and hospitalization time were shorter than those in group A,the differences were statistically significant(P<0.05).Three months after operation,the levels of von Willebrand factor(vWF)and endothelin-1(ET-1)in two groups decreased,and the level of nitric oxide(NO)in two groups increased,the levels of vWF and ET-1 in group B were lower than those in group A,and the level of NO was higher than that in group A,the differences were statistically significant(P<0.05).The complication rate of group B was 7.32%,which was significantly lower than 23.81%of group A,and the difference was statistically significant(P<0.05).Conclusion:Compared with microsurgical clipping,endovascular intervention can achieve better effects in the treatment of patients with intracranial aneurysms,with less vascular endothelial damage.
