脉冲CO_(2)激光结合牙周基础治疗对青春期龈炎的牙菌斑抑制效果及患者生活质量的影响

摘　　要：目的观察脉冲CO_(2)激光结合牙周基础治疗对青春期龈炎的牙菌斑抑制效果及对患者生活质量的影响。方法选择2021年2月至2022年2月就诊的青春期龈炎患者72例,按照系统随机化法均分为2组,每组36例。对照组行牙周基础治疗,治疗组行脉冲CO_(2)激光结合牙周基础治疗,比较2组治疗前后菌斑指数、牙龈指数、出血指数、探诊深度、菌斑百分率、口臭评分、牙龈疼痛程度、肿胀程度及口腔健康影响程度量表(OHIP-14)评分的变化。2组均于治疗后6周时按照标准统计临床疗效。结果2组菌斑指数、牙龈指数、出血指数、探诊深度、菌斑百分率、口臭评分、牙龈疼痛程度、肿胀程度均较治疗前降低,且治疗组低于对照组(P<0.05)。2组生活质量口腔功能受限、口腔疼痛、心理不适、社交障碍评分和总分均较治疗前下降,治疗组各项评分及总分均低于对照组(P<0.05)。治疗组临床治愈率为80.56%高于对照组的63.89%(P<0.05)。结论脉冲CO_(2)激光结合牙周基础治疗对青春期龈炎进行治疗能有效抑制牙菌斑,改善患者牙周症状,提高患者生活质量。Objective To observe the effect of pulsed CO_(2) lasers combined with periodontal initial therapy in inhibiting the dental plaque and improving the quality of life of patients with puberty gingivitis.Methods Seventy-two patients with puberty gingivitis treated from February 2021 to February 2022 were recruited,they were divided into 2 groups according to the systematic randomization method.Thirty-six patients of the control group were treated with periodontal initial therapy,and 36 patients of the treatment group were treated with pulsed CO_(2) lasers combined with periodontal initial therapy.The changes of plaque index,gingival index,bleeding index,probing depth,percentage of dental plaque,halitosis score,gingival pain and swelling,and the Oral Health Impact Profile-14(OHIP-14)score before and after treatment were compared between the two groups.The clinical curative effect of both groups was recorded at 6 weeks after treatment.Results After treatment,the plaque index,gingival index,bleeding index,probing depth,percentage of dental plaque,halitosis score,gingival pain and swelling degree in the two groups were all significantly lower than those before treatment,which were significantly lower in the treatment group than in the control group(P<0.05).After treatment,the oral function limitation,oral pain,psychological discomfort,social barrier scores and total scores of the quality of life in the two groups all significantly decreased compared with those before treatment,which were significantly lower in the treatment group than those in the control group(P<0.05).The clinical curative rate in the treatment group was significantly higher than that in the control group(80.56%vs 63.89%,P<0.05).Conclusion The pulsed CO_(2) lasers combined with periodontal initial therapy can effectively inhibit the dental plaque,alleviate periodontal symptoms and improve the quality of life of patients with puberty gingivitis.
