FS-LASIK与SMILE术后眼干燥症发生情况的对比分析

摘　　要：目的:分析对比飞秒激光辅助准分子激光角膜原位磨镶术(FS-LASIK)和小切口飞秒激光基质透镜取出术(SMILE)术后眼干燥症的发生情况。方法:选取2022年5月—11月在江苏省镇江市第四人民医院接受近视激光手术的88例患者的临床资料,依据治疗方式的不同将其分为FS-LASIK组(n=40)和SMILE组(n=48)。比较两组术前、术后1周、术后1个月和术后3个月的眼表疾病指数(OSDI)评分、基础泪液分泌实验Ⅰ(SIT)、泪膜破裂时间(BUT)、角膜荧光素染色(FL)检查情况,术后1个月和3个月的眼干燥症发生率。结果:两组术前、术后1周及术后3个月眼干燥症的发生率差异无统计学意义(P>0.05);两组术前、术后1周、1个月OSDI指数比较,差异无统计学意义(P>0.05),术后3个月OSDI指数比较,差异具有统计学意见(P<0.05);两组术后1周、1个月泪液分泌量比较,差异有统计学意义(P<0.05),两组术前、术后3个月泪液分泌量比较,差异无统计学意义(P>0.05);术后1周、1个月和3个月FS-LASIK组泪膜破裂时间均短于SMILE组,差异均具有统计学意义(P<0.05)。术后1周、1个月和3个月FS-LASIK组角膜荧光素染色评分均高于SMILE组,差异均具有统计学意义(P<0.05)。结论:FS-LASIK和SMILE两种术式术后均会出现干眼表现,但SMILE术后眼干燥症状和体征均较轻,效果好于FS-LASIK术。Objective To observe the dry eye after femtosecond laser-assisted in situ keratomileusis(FS-LASIK)and small incision femtosecond laser matrix lens extraction(SMILE).Methods Clinical data of 88 patients who underwent myopia laser surgery at the Fourth People’s Hospital of Zhenjiang City,from May to November 2022 were selected and divided into FS-LASIK surgery group(n=40)and SMILE surgery group(n=48)based on different treatment methods.The ocular surface disease index(OSDI)score,basic tear secretion testⅠ(SIT),tear film rupture time(BUT),corneal fluorescein staining(FL)and the incidence rate of xerophthalmia 1 month and 3 months after surgery were compared between the two groups.Results There was no statistically significant difference in the incidence of dry eye before 1 month and after 3 months after surgery in the FS-LASIK group(P>0.05),There was no statistically significant difference in the OSDI index between the FS-LASIK group at 1 week and 1 month after surgery(P>0.05),but there was a statistically significant difference in the OSDI index at 3 months after surgery(P<0.05);There was no statistically significant difference in tear secretion between the FS-LASIK group at 1 week and 1 month after surgery(P<0.05),and there was no statistically significant difference in tear secretion at 3 months after surgery(P>0.05);The tear film rupture time in the FS-LASIK group was shorter than that in the SMILE group at 1 week,1 month,and 3 months after surgery,and the differences were statistically significant(P<0.05).The corneal fluorescence staining scores of the FS-LASIK group were higher than those of the SMILE group at 1 week,1 month,and 3 months after surgery,and the differences were statistically significant(P<0.05).Conclusion Both FS-LASIK and SMILE procedures may present dry eye symptoms after surgery,but SMILE has mild dry eye symptoms and signs,resulting in better results than FS-LASIK.
