摘　　要：目的探讨不同HBV基因型乙肝相关肝细胞癌患者的临床特征和血管内皮生长因子(VEGF)及其受体(Flt-1,Flk-1)在肝癌组织中的表达。方法前瞻性连续纳入2020年10月至2022年10月在郑州大学第一附属医院就诊住院的乙肝相关肝细胞癌患者200例作为研究对象,根据患者HBV基因型检测结果将患者分为HBV基因型B型组101例,HBV基因型C型组90例,HBV基因型D型组9例,分析肝功能相关指标以及VEGF和受体(Flt-1,Flk-1)的表达的差异。结果HBV基因型C型肝细胞癌患者甲胎蛋白(AFP)水平以及HBV载量、Child-Pugh评分、血管内皮生长因子(VEGF)及其受体(Flt-1,Flk-1)表达较HBV基因型B型肝细胞癌与HBV基因型D型肝细胞癌更高。结论不同HBV基因型患者的肿瘤标志物、病毒载量以及肝功能比较有区别,对于确诊的肝细胞癌患者及时进行HBV基因型检测和VEGF及其受体(Flt-1,Flk-1)表达检测对于治疗以及预后具有指导意义。Objective To investigate the clinical features and expression of vascular endothelial growth factor(VEGF)and its receptor(Flt-1,Flk-1)in hepatocellular carcinoma(HCC)patients with different HBV genotypes.Methods A total of 200 patients with hepatitis B associated hepatocellular carcinoma admitted to the First Affiliated Hospital of Zhengzhou University from October 2020 to October 2022 were prospectively enrolled as the research objects.According to the results of HBV genotype detection,the patients were divided into HBV genotype B group 101 cases,HBV genotype C group 90 cases and HBV genotype D group 9 cases.The differences of liver function and expression of VEGF and its receptor(Flt-1,Flk-1)were analyzed.Results The level of alpha-fetoprotein(AFP),HBV load,Child-Pugh score and expression of VEGF and its receptor(Flt-1,Flk-1)in HBV genotype C hepatocellular carcinoma patients were higher than those in HBV genotype B and HBV genotype D hepatocellular carcinoma patients.Conclusion There are differences in tumor markers,viral load and liver function among patients with different HBV genotypes.Timely detection of HBV genotype and expression of VEGF and its receptor(Flt-1,Flk-1)in patients with confirmed hepatocellular carcinoma is of guiding significance for treatment and prognosis.
