口腔种植治疗中引导骨再生手术联合浓缩生长因子的作用

摘　　要：目的 分析将引导骨再生手术(guided bone regeneration,GBR)+浓缩生长因子(concentrate growth factors,CGF)用于口腔种植治疗中的临床价值。方法 选取2020年1月—2022年12月苏州口腔医院收治的100例口腔种植患者为研究对象,按照随机数表法分为对照组和观察组,每组50例。对照组植入Bio-Oss骨粉并覆盖海奥生物膜,观察组在对照组基础上制备CGF,联合CGF治疗;两组均行GBR。对比两组种植成功率、炎症反应、骨吸收指标、不良反应发生率及黏膜愈合情况。结果 观察组种植成功率100.00%高于对照组的92.00%,但差异无统计学意义(χ^(2)=2.344,P>0.05)。观察组人高迁移率族蛋白B1、细胞间粘附分子-1低于对照组,程序性死亡受体-配体1、人β防御素3均高于对照组,差异有统计学意义(P<0.05)。观察组骨钙素、骨保护素高于对照组,I型胶原氨基端肽低于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。观察组黏膜颜色、黏膜肿胀程度评分均低于对照组,差异有统计学意义(P<0.05)。结论 将GBR+CGF联合用于口腔种植治疗中可发挥积极作用,能够提升种植成功率,减轻龈沟炎症反应,改善骨吸收状态,且不良反应少,黏膜愈合良好。Objective To analyze the clinical value of guided bone regeneration(GBR) combined with concentrate growth factors(CGF) in oral implantation therapy.Methods A total of 100 patients with oral implant admitted to Suzhou Stomatological Hospital from January 2020 to December 2022 were selected as the research objects.According to the random number table method,they were divided into control group and observation group,with 50 cases in each group.The control group was implanted with Bio-Oss bone powder and covered with Haiao biofilm.The observation group was prepared with CGF on the basis of the control group and combined with CGF treatment.Both groups underwent GBR.The success rate of implantation,inflammatory response,bone resorption index,incidence of adverse reactions and mucosal healing were compared between the two groups.Results The success rate of implantation in the observation group was 100.00%,higher than that in the control group(92.00%),but the difference was not statistically significant(χ^(2)=2.344,P<0.05).The levels of human high-mobility group protein box-1(HMGB1) and intercellular adhesion molecule-1(ICAM-1) in the observation group were lower than those in the control group,the programmed cell death-ligand 1(PD-L1) and human beta-defensin-3(HBD-3) were higher than those in the control group,and the difference was statistically significant(P<0.05).Bone gla-protein(BGP) and osteoprotegerin(OPG) in the observation group were higher than those in the control group,and N-terminal telopeptide of type I collagen(NTX) was lower than that in the control group,the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).The scores of mucosal color and mucosal swelling in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion The combination of GBR and CGF can play a positive role in oral
