摘 要:目的 探讨糖尿病视网膜病变(diabetic retinopathy,DR)并发白内障患者白内障术后瞳孔改变情况,并分析相关影响因素。方法 收集2018年2月—2023年1月在广州医科大学附属第二医院住院治疗的DR并发白内障患者72例(72眼),均行白内障超声乳化摘除术+人工晶体植入术,术后依据是否发生瞳孔改变分为两组,将瞳孔未改变的患者[32例(32眼)]作为对照组,瞳孔改变的患者[40例(40眼)]作为研究组,统计两组临床资料,并就DR并发白内障患者白内障术后瞳孔改变的影响因素进行多元Logistic回归分析。结果 两组年龄、性别、糖化血红蛋白经对比,差异无统计学意义(P>0.05),而糖尿病病程、DR严重程度对比,差异有统计学意义(P<0.05)。经Logistic回归分析得知,糖尿病病程、DR严重程度是DR并发白内障患者白内障术后发生瞳孔改变的主要影响因素(P<0.05)。结论 DR严重程度、糖尿病病程是对DR并发白内障患者术后瞳孔改变造成影响的主要因素。Objective To explore the post-cataract pupillary alterations in cataract patients with diabetic retinopathy(DR) complicated by cataract,and to analyze the related influencing factors.Methods A total of 72 patients(72 eyes)with cataract complicated by DR Who were hospitalized in the Second Affiliated Hospital of Guangzhou Medical University from February 2018 to January 2023 were collected,all of whom underwent phacoemulsification and intraocular lens implantation.They were divided into two groups according to whether pupil changes occurred after surgery.Patients with unchanged pupils [32 cases(32 eyes)] were used as control group and patients with altered pupils [40 cases(40 eyes)] as study group.The clinical data of the two groups were analyzed,and multiple Logistic regression analysis was performed on the factors affecting the change of pupils after cataract operation in patients with cataract complicated by DR.Results There were no statistically significant differences in age,gender and HbA1c between the two groups(P>0.05),but there were statistically significant differences in diabetes course and DR Severity between the two groups(P<0.05).Logistic regression analysis showed that the duration of diabetes and the severity of DR Were the main factors affecting the change of pupils after cataract surgery in patients with cataract complicated by DR(P<0.05).Conclusion DR severity and diabetes duration are the main factors affecting postoperative pupillary alterations in cataract patients with DR.
