摘　　要：目的 探讨阿加曲班联合双重抗血小板对急性进展性脑梗死的疗效及对血浆血栓塞B2(TXB2)、6-酮-前列腺素F1α(6-keto-PGF1α)水平影响。方法 选取2020年2月-2021年5月漯河市中心医院88例急性进展性脑梗死患者,随机分为对照组(n=43)与研究组(n=43)。对照组给予双重抗血小板聚集治疗,研究组在双重抗血小板基础上联合阿加曲班治疗。比较治疗后两组神经功能缺损、疗效、血浆TXB2、6-keto-PGF1α水平及不良反应。结果 治疗后研究组NIHSS评分低于对照组(P<0.05)。研究组有效率高于对照组(P<0.05)。治疗后研究组TXB2水平低于对照组、6-keto-PGF1α水平高于对照组(P<0.05)。两组不良反应无统计学意义(P>0.05)。结论 阿加曲班联合双重抗血小板治疗可有效降低急性进展性脑梗死患者神经功能缺损,改善症状,降低血浆中TXB2含量、提高6-keto-PGF1α表达,无明显严重出血等不良反应,推荐临床治疗使用。Objective To investigate the efficacy of argatroban combined with dual-antiplatelet therapy in the treatment of acute progressive cerebral infarction,and to analyze its effect on plasma thromboxan B2(TXB2)and 6-keto-prostaglandin Flα(6-Keto-PGFlα).Methods Eighty-eight patients with acute progressive cerebral infarction admitted to Luohe Central Hospital from February 2020 to May 2021 were randomly divided into two groups,each with 43 cases.Control group was treated with dual-antiplatelet therapy,while study group was treated with dual-antiplatelet therapy combined with argatroban.Then the National Institute of Health Stroke Scale(NIHSS)scores,clinical efficacy,plasma levels of TXB2 and 6-Keto-PGFlα,adverse reaction were compared between after treatment.Results After treatment,NIHSS score was significantly decreased in both groups,and the NIHSS score of study group was lower than that of control group(P<0.05).The effective rate of study group was higher than that of control group(P<0.05).Compared with control group,study group had significantly lower TXB2 level as well as higher 6-Keto-PCFlαlevel(P<0.05).The adverse reactions of the two groups were not statistically significant(P>0.05).Conclusion Argatroban combined with dual-antiplatelet therapy can effectively alleviate neurological impairment,ameliorate the symptoms and signs,reduce plasma TXB2 level and increase the expression of 6-Keto-PGF1αin patients with acute progressive cerebral infarction,and it is recommended for clinical treatment.
