缩瞳剂在慢性闭角型青光眼中使用效果及对房水炎性指标影响

摘　　要：目的 探讨缩瞳剂在慢性闭角型青光眼(ACG)中使用效果及对房水炎性指标影响。方法 选取我院2019年7月-2020年1月收治的56例(60眼)CACG患者作为研究对象,随机数字表法均分为试验组(28例,31眼)和对照组(28例,29眼)。两组均接受小梁切除术治疗,对照组术后给予左氧氟沙星滴眼液,试验组在对照组的基础上再给予缩瞳剂毛果芸香碱滴眼液,两组均治疗1个月。比较两组用药前、用药1个月后眼压(IOP)、视野平均缺损(MD)、模式标准差(PSD)、房水炎症因子白细胞介素(IL)-6、干扰素(INF)-γ、治疗过程中感染、浅前房、结膜充血等不良反应发生情况。结果 用药前两组IOP差异无统计学意义(P>0.05);用药1个月后,试验组IOP低于对照组(P<0.05);用药前及用药1个月后试验组与对照组视野MD、PSD差异均无统计学意义(P>0.05);用药前,两组房水IL-6、INF-γ含量差异不具有统计学意义(P>0.05);用药1个月后试验组房水IL-6、INF-γ含量均低于对照组(P<0.05);试验组总不良反应发生率为6.45%(2例/31例),对照组为13.79%(4例/29例),两组总不良反应发生率差异不具有统计学意义(P>0.05)。结论 缩瞳剂毛果芸香碱能够降低CACG患者眼压及房水炎症因子含量,不增加不良反应,值得临床推广。Objective To investigate the effect of miotic agents in chronic angle-closure glaucoma(CACG)and their influence on inflammatory indexes of aqueous humor.Methods A total of 56 patients(60 eyes)with CACG admitted to our hospital from July 2019 to January 2020 were selected as the research objects.They were divided into the test group(28 cases,31 eyes)and the control group(28 cases,29 eyes)by random number table method.Both groups received trabeculectomy,the control group was given levofloxacin eye drops after the operation,and the test group was given the miotic agent pilocarpine eye drops on the basis of the control group.Both groups were treated for 1 month.The intraocular pressure(IOP),mean visual field defect(MD),model standard deviation(PSD),aqueous inflammatory factors interleukin(IL)-6,interferon(INF)-ybefore and 1 month after treatment between the two groups,the occurrence of adverse reactions such as infection,shallow anterior chamber,conjunctival hyperemia during treatment.Results There was no significant difference in IOP between the two groups before treatment(P>0.05);After 1 month of treatment,the IOP of the test group was lower than that of the control group(P<0.05);The visual field of the test group and the control group before and 1 month after treatment.The differences in MD and PSD were not statistically significant(P>0.05);Before treatment,the differences in the levels of IL-6 and INF-γ in aqueous humor between the two groups were not statistically significant(P>0.05);The contents of IL-6 and INF-γ in water were reduced(P<0.05);The total adverse reaction rate in the experimental group was 6.45%(2 cases/31 cases),and the control group was 13.79%(4 cases/29 cases).The difference in the incidence of total adverse reactions was not statistically significant(P>0.05).Conclusion The miotic agent pilocarpine can reduce the intraocular pressure and the content of inflammatory factors in the aqueous humor of CACG patients,without increasing adverse reactions,and is worthy of clinical promotion.
