摘 要:目的:综述维生素D在急性髓系白血病(AML)中的作用及机制研究进展,为AML的治疗提供新思路。方法:采用文献综述的方法,系统地搜集和分析近年来有关维生素D在AML中的相关研究成果。结果:总体来说,AML的治愈率相对较低,预后不佳,老年和体弱患者的治疗成功率不理想,主要是因为这些患者的身体状况较差,化疗耐受性差,同时也可能存在其他疾病和并发症的影响。维生素D对AML的作用包括促进AML细胞的分化、增强免疫功能、影响AML细胞的代谢途径等。维生素D可以通过与维生素D受体(VDR)结合,调节多个信号通路,如Wnt、NF-kappaB等,从而影响AML细胞的分化和凋亡。维生素D还可以促进巨噬细胞的活化和自然杀伤细胞的增殖和活化,从而增强机体的免疫功能,抑制AML细胞的生长和扩散。结论:维生素D参与了AML的发生和发展过程。维生素D能够抑制AML白血病干细胞增殖并诱导其分化,从而降低AML的发病率和提高治疗效果。此外,维生素D还通过调节T细胞免疫应答、影响转录因子、基因表达等途径,在AML辅助治疗中具有重要作用。【Objective】To review the role and mechanism of vitamin D in acute myeloid leukemia(AML)to provide new ideas for the treatment of AML.【Method】This paper used the literature review method to systematically collect and analyze the recent studies of vitamin D in AML.【Result】Overall,AML has a relatively low cure rate and a poor prognosis.The unsatisfactory success rate in elderly and frail patients is mainly because of their poor physical condition,poor tolerance of chemotherapy,and the influence of other diseases and complications.The effects of vitamin D on AML include promoting the differentiation of AML cells,enhancing the immune function,and influencing the metabolic pathways of AML cells.Vitamin D can bind with vitamin D receptor(VDR)and regulate multiple signaling pathways,such as Wnt and NF-kappaB,thus affecting the differentiation and apoptosis of AML cells.Vitamin D can also promote the activation of macrophages and the proliferation and activation of natural killer cells,thus enhancing the immune function of the body and inhibiting the growth and spread of AML cells.【Conclusion】Vitamin D is involved in the development and development of AML.Vitamin D is able to inhibit the proliferation and induce the differentiation of AML leukemia stem cells,thus reducing the incidence of AML and improving the therapeutic effect.In addition,vitamin D also has an important role in AML treatment by regulating T cell immune response,affecting transcription factors and gene expression.
