眼睑痉挛A型肉毒杆菌毒素注射不同点位的效果研究

摘　　要：目的探讨眼睑痉挛采用A型肉毒杆菌毒素不同点位注射治疗的效果。方法将60例眼睑痉挛患者随机分为2组,Ⅰ组30例,接受筋膜前注射;Ⅱ组30例,接受睑板前注射。注射前和注射3个月后评定痉挛强度分级,评估注射后起效时间、有效时间及并发症。结果治疗前2组患者眼睑痉挛程度分级比较,差异无统计学意义(P>0.05);治疗3个月后Ⅱ组痉挛程度分级优于Ⅰ组,差异有统计学意义(P<0.05);起效时间Ⅱ组快于Ⅰ组,差异有统计学意义(P<0.05);有效时间Ⅱ组长于Ⅰ组,差异有统计学意义(P<0.05);上睑下垂的发生率Ⅱ组低于Ⅰ组,差异有统计学意义(P<0.05)。结论眼睑痉挛患者睑板前注射A型肉毒杆菌毒素可有效改善痉挛程度,且起效时间更快、有效时间更长、上睑下垂情况发生更少。Objective To study effects of injection site A botulinum toxin in treatment of eyelid dystonia.Methods 60 patients in eyelid dystonia were divided into two groups.Ⅰgroup 30 cases,was given preseptal injection treatment.Ⅱgroup 30 cases,was given pretarsal injection treatment.The evalution of cohen spasm classification was done in before injection,the three months after injection treating.The evalution of the onset time,the useful time and complication were done in after injection treating.Results Cohen spasm classification in two group was no significant difference in before injection treating(P>0.05).The three months after injection treating,cohen spasm classification in two group was obviously superior to those of before injection(P<0.05),the relieving spasmsⅡgroup was better than thatⅠgroup(P<0.05).The onset time ofⅡgroup was faster than thatⅠgroup(P<0.05).The useful time ofⅡgroup was longer than thatⅠgroup(P<0.05).The rate of blepharoptosis inⅡgroup was belower than that inⅠgroup(P<0.05).Conclusion Cohen spasm classification in two group was significant effct in after injection treating.The onset time inⅡgroup was faster than that inⅠgroup A.The useful time inⅡgroup was longer than that inⅠgroup.Blepharoptosis inⅡgroup was belower than that inⅠgroup.
