摘　　要：慢性阻塞性肺疾病(COPD)合并肺癌的患者在临床上并不罕见,随着对COPD的深入研究,有研究表明肺部存在特定的微生物群,并且COPD共患其他肺部疾病的患者肺部微生物群多样性变化更加显著。众多研究表明,肺癌合并COPD患者的疾病进展尤为迅速。因此,研究COPD合并肺癌患者的肺部微生物群变化、免疫炎症状态以及临床表型的改变与其预后的相关性,对于有效干预疾病进展具有重要的临床意义。这篇综述整合了关于COPD、肺癌、COPD共患肺癌三种疾病状态下的炎症状态、肺部微生态以及临床表型的研究发现,可进一步确定COPD及肺癌共患状态时微生物群对于疾病进展的作用,同时验证微生物群及其代谢产物是否可以作为疾病进展或不良预后的生物标志物,从而为确定新的潜在治疗靶点提供新的思路。Patients with chronic obstructive pulmonary disease(COPD)complicated with lung cancer are not uncommon in clinic practice.Specific microbiota exist in the lungs,and the diversity of pulmonary microbiota is more pronounced in COPD patients combined with other pulmonary diseases.Numerous studies have found that the disease progression of COPD combined with lung cancer is particularly rapid,and the prognosis of patients with moderate or more severe COPD and frequent exacerbation is worse.Therefore,the correlation study of the changes of lung microboita,the changes of immune inflammatory state,and clinical phenotype and its prognosis is of great clinical significance to develop an effective intervention alleviating disease progression.This review integrates findings on inflammatory states,pulmonary microecology,and clinical phenotypes in COPD,lung cancer,and COPD combined with lung cancer.It aims to determine the role of microbiota in disease progression of moderate or severe COPD and frequent exacerbations of lung cancer,and verify whether microbiota and its metabolites can be used as prognostic biomarkers,thus providing new potential therapeutic targets.
