肾脏影像学检查肾细胞癌患者和健康者尿酸血尿的对比分析

摘　　要：目的对比分析高尿酸、血尿在肾细胞癌(RCC)患者与肾脏影像学检查健康者之间的差异性。方法回顾性分析206例经手术病理证实为RCC患者术前血尿酸水平及尿红细胞个数,采取配对设计随机抽样选取206例肾脏影像学检查健康者作对照,分析2组人群间高尿酸、血尿的差异。结果RCC与肾脏影像学检查健康者之间的高尿酸和血尿差异有统计学意义(P<0.05)。多因素分析采用Logistic回归结果显示有血尿的人群患RCC的风险是无血尿的4.988倍[95%CI(1.384,17.971),P=0.014]。高尿酸人群患RCC的风险是尿酸正常的2.686倍[95%CI(1.757,4.106),P<0.001]。结论高尿酸和血尿在肾细胞癌患者中较健康人群容易出现,对肾细胞癌的诊断具有一定的提示意义。Objective To compare the differences in high uric acid and hematuria between patients with renal cell carcinoma(RCC)and normal people.Methods Two hundred and six patients with RCC confirmed by surgical pathology were retrospectively analyzed for preoperative the level of uric acid in the blood and the number of red blood cells in urine,206 patients with normal renal imaging were randomly selected by paired design as control.The differences in high uric acid and hematuria between the two groups were analyzed.Results Univariate analysis showed statistically significant differences in high uric acid and hematuria between RCC and the normal population.The results of logistic regression used for multivariate analysis showed that the risk of RCC in people with high uric acid was 2.686 times more likely to develop RCC than those with normal uric acid[95%CI(1.757,4.106),P<0.001].People with hematuria were 4.988 times that those without hematuria[95%CI(1.384,17.971),P=0.014].Conclusion High uric acid and hematuria are more common in patients with RCC than in the normal population,which has certain suggestive significance for the diagnosis of renal cell carcinoma.
