摘 要:目的分析甲状腺乳头状癌(PTC)全切术后患者在停用左旋甲状腺激素(LT4)所致的甲状腺功能减退对天门冬氨酸氨基转移酶(ALT)和丙氨酸氨基转移酶(AST)的影响。方法回顾性分析PTC术后行131I治疗的患者,记录术前、停用LT4和补充LT4时血清ALT、AST和游离三碘甲状腺原氨酸(FT3)、游离四碘甲状腺原氨酸(FT4)的水平,并进行相关性分析。结果对比术前,停用LT4后血清ALT和AST显著升高,补充LT4后显著降低,且3组间ALT和AST有统计学意义,P<0.01。(2)FT4、FT3分别与^(Δ)ALT1(术前与停用LT4的ALT差值)、^(Δ)ALT2(停用与补充LT4的ALT差值)呈负相关(r=-0.251,-0.257,-0.299,-0.277;P均<0.05)。(3)FT4、FT3分别与^(Δ)AST1(术前与停用LT4的AST差值)、^(Δ)AST2(停用与补充LT4的AST差值)呈负相关(r=-0.415;r=-0.416;r=-0.395;r=-0.403;P均<0.01)。结论甲状腺功能减退会影响血清ALT和AST。甲状腺激素与血清ALT、AST呈负相关。Objective To analyze the effect of hypothyroidism caused by withdrawal of levothyroxine(LT4)on ALT and AST in patients with papillary thyroid carcinoma(PTC)after total thyroidectomy.Methods Retrospectively analyze patients with PTC who underwent 131I treatment after surgery.Record serum ALT,AST,FT3,and FT4 levels before,during and after LT4 withdrawal,and correlatively analyze the data.Results①Compared with before surgery,serum ALT and AST increased significantly after LT4 withdrawal and decreased significantly after LT4 supplementation,and there was a statistical significance between the three groups of ALT and AST,P<0.01.②FT4 and FT3 were negatively correlated with^(Δ)ALT1(the difference in ALT between before surgery and after withdrawal of LT4)and^(Δ)ALT2(the difference in ALT between withdrawal and supplementation of LT4),respectively(r=-0.251,-0.257,0.299,-0.277;P<0.05 for all).③FT4 and FT3 were negatively correlated with^(Δ)AST1(the difference in AST between before surgery and after withdrawal of LT4)and^(Δ)AST2(the difference in AST between withdrawal and supplementation of LT4),respectively(r=-0.415,-0.416,-0.395,-0.403;P<0.01 for all).Conclusion Hypothyroidism affects serum ALT and AST.Thyroid hormone is negatively correlated with serum ALT and AST.
