摘  要:目的 分析门冬氨酸鸟氨酸对亚临床肝性脑病(SHE)患者肝功能、智力水平及氧化应激反应水平的影响机制。方法 选取2021年1月至2022年4月97例SHE患者,以计算机随机分组法将其分为2组,对照组48例,给予精氨酸治疗,观察组49例,给予门冬氨酸鸟氨酸治疗,比较2组肝功能、智力、氧化应激反应、肝性脑病和不良反应发生情况。结果 观察组丙氨酸转氨酶、总胆红素、天冬氨酸转氨酶低于对照组(P<0.05);观察组数字连接试验A、数字连接试验B、线追踪试验、数字符号试验用时均短于对照组(P<0.05);观察组丙二醛、内皮素-1低于对照组,超氧化物歧化酶、一氧化氮高于对照组(P<0.05);观察组发生肝性脑病较对照组少(P<0.05);2组不良反应发生率对比差异无统计学意义(P>0.05)。结论 门冬氨酸鸟氨酸不仅能够有效调节SHE患者氧化应激水平,减轻肝脏损伤,降低肝性脑病发生率,还能提高患者智力水平,且不增加不良反应发生率。Objective To analyze the mechanism underlying the effects of ornithine aspartate on liver function,intelligence and oxidative stress response in patients with subclinical hepatic encephalopathy(SHE).Methods Between January 2021 and April 2022,a total of 97 SHE patients were included and divided into two groups by computer program.The control group(n=48)was treated on arginine,and the study group(n=49)was treated on ornithine aspartate.Then the two groups were compared for liver function,intelligence,oxidative stress response,hepatic encephalopathy and adverse reactions.Results The study group showed lower levels of ala-nine aminotransferase(ALT),total bifirubin(TBIL)and aspartate aminotransferase(AST)compared with the control group(P<0.05).The study group spent shorter time to completion of number connection test A(NCT-A),number connection test B(NCT-B),line tracing test(LTT),and digital symbol test(DST)than did the control group(P<0.05).Compared with the control group,the study group showed lower levels of malondialdehyde(MDA)and endothelin-1(ET-1),but higher levels of superoxide dismutase(SOD)and nitric oxide(NO)(P<0.05).The occurrence of hepatic encephalopathy in the study group was less common than that in the control group(P<0.05).There was no significant difference in adverse reactions between groups(P>0.05).Conclusion In patients with SHE,ornithine aspartate can effectively regulate the level of oxidative stress,ameliorate liver damage,reduce the incidence of hepatic encephalopathy.Other than these,it also improves the intelligence in the patients without in-crease in the incidence of adverse reactions.
