双侧改良Bernard-Webster瓣保留口角轴修复下唇缺损的临床研究

摘　　要：目的:探索改良Bernard-Webster瓣修复下唇缺损的临床效果。方法:收集2016年6月~2021年6月南京市口腔医院收治的12例因恶性肿瘤手术切除导致下唇缺损的患者,应用双侧改良Bernard-Webster瓣修复,术后随访12~37个月,观察修复后的下唇功能、形态恢复及神经感觉功能情况。结果:12例下唇癌患者术后即刻修复重建,肿瘤切除后下唇缺损均超过2/5,缺损最大者达下唇4/5。患者均未出现术后伤口裂开、感染或皮瓣坏死等局部并发症。12个月后2例患者中线处红唇沟状凹陷,瘢痕增粗,所有患者口腔功能、语音、感觉均恢复良好,无复发或死亡病例,均对治疗结果较满意。结论:应用双侧改良Bernard-Webster瓣修复下唇缺损有利于恢复下唇的形态和功能,值得临床推广。Objective:To explore the clinical effect of modified Bernard-Webster flap on the repair of lower lip defects.Methods:From June 2016 to June 2021,a total of 12 patients with lower lip defects caused by surgical resection of malignant tumors were recorded and repaired by bilateral modified Bernard-Webster flap in our department.The patients were followed up for 12 to 37 months to observe the restoration of the function,shape and sensation of the lower lip after repair.Results:Twelve patients with lower lip cancer whose lip defect exceeded 2/5 and the largest defect reached 4/5 underwent immediate reconstruction after tumor excision.None of them had local complications,such as wound dehiscence,infection and flap necrosis.The function,speech and sensation of all patients recovered well with no recurrence and death occurred while the scars of lower lip were hyperplasia and depressed in two patients after 12 months.Conclusion:The application of the bilateral modified Bernard-Webster flap to repair the defect of the lower lip is more conducive to the restoration of the shape and function of the lower lip,and it is worthy of clinical promotion.
