摘 要:目的分析表观弥散系数(ADC)值联合宝石能谱计算机断层扫描(CT)成像技术对食管癌病理分级的诊断价值。方法选取我院2020年2月至2021年3月收治的93例食管癌患者为研究对象,根据影像学资料分为高分化组(n=26)、中分化组(n=32)和低分化组(n=35)。三组患者均在术前接受胸部宝石能谱CT成像技术及磁共振成像(MRI)扫描检查。比较三组的ADC值、动脉期强化值(HUA)、静脉期强化值(HUV)、动脉期标准化碘浓度(NICA)、静脉期标准化碘浓度(NICV)值;分析ADC值联合HUA、HUV、NICA、NICV值对食管癌病理分级的诊断价值。结果高分化组的ADC值高于中分化组和低分化组,HUA、HUV、NICA、NICV值均低于低分化组,差异具有统计学意义(P<0.05)。受试者工作特征(ROC)曲线结果显示,ADC、NICA诊断食管癌病理分级的曲线下面积(AUC)>0.8,具有较高诊断效能。ADC联合NICA诊断食管癌病理分级的AUC、灵敏度、特异度分别为0.906、0.853、0.831,其诊断价值最高。结论ADC值联合宝石能谱CT成像技术对食管癌病理分级具有较高诊断效能,可为患者的及时治疗提供更加准确的影像学信息,值得临床推广。Objective To analyze the diagnostic value of apparent diffusion coefficient(ADC)value combined with gemstone spectral computed tomography(CT)imaging technology in pathological grading of esophageal cancer.Methods A total of 93 patients with esophageal cancer admitted in our hospital from February 2020 to March 2021 were selected as the research objects.According to the imaging data,the patients were divided into high differentiation group(n=26),middle differentiation group(n=32)and low differentiation group(n=35).All three groups of patients underwent chest gemstone epectral CT imaging technology and magnetic resonance imaging(MRI)scans before surgery.The ADC value,enhancement value in arterial phase(HUA),enhancement value in venous phase(HUV),normalized iodine concentration in arterial phase(NICA)and normalized iodine concentration in venous phase(NICV)were compared among the three groups;the diagnostic value of ADC value combined with HUA,HUV,NICA and NICV in pathological grading of esophageal cancer were analyzed.Results The ADC value of the high differentiated group was higher than that of the middle differentiation group and the low differentiation group,the values of HUA,HUV,NICA and NICV were lower than those of the low differentiation group,and the differences were statistically significant(P<0.05).The results of receiver operating characteristic(ROC)curve showed that the area under curve(AUC)of ADC and NICA in the diagnosis of pathological grading of esophageal cancer was>0.8,which had high diagnostic efficiency.The AUC,sensitivity and specificity of ADC combined with NICA in the diagrosis of pathological grading of esophageal cancer were 0.906,0.853 and 0.831,respectively,which had the highest diagnostic value.Conclusion ADC value combined with gemstone spectral CT imaging technology has high diagnostic efficiency for pathological grading of esophageal cancer,it can provide more accurate imaging information for timely treatment of patients with esophageal cancer,which is worthy of clinical promotion.
