功能性矫治器对幼年特发性关节炎引起的颞下颌关节炎治疗效果的meta分析

摘　　要：目的分析功能性矫治器对幼年特发性关节炎(JIA)引起的颞下颌关节炎的临床疗效。方法在Web of Science、Cochrane Library、PubMed、Embase、维普、万方数据库、中国知网、中国生物医学文献数据库等检索功能性矫治器治疗JIA引起的颞下颌关节炎的临床研究,检索时限为建库至2022年5月31日。由两名研究人员根据纳入和排除标准筛选文献,提取纳入文献的基本信息及数据,并评价文献的偏倚风险,应用RevMan5.4软件进行meta分析。结果共纳入10篇文献,529例JIA引起的颞下颌关节炎患者。meta分析结果显示:功能性矫治器能够缓解JIA引起的颞下颌关节炎患者的颌面部疼痛(OR=0.24,95%CI:0.14~0.42,P<0.05),改善张口度(MD=11.73,95%CI:8.78~14.69,P<0.05),减少颞下颌关节的关节弹响(OR=0.32,95%CI:0.18~0.58,P<0.05)。结论功能性矫治器能够有效改善JIA引起的颞下颌关节炎的颌面部症状,值得临床推广。Objective To investigate the clinical effect of the functional appliance on temporomandibular arthritis(TMA)caused by juvenile idiopathic arthritis(JIA).Methods A comprehensive search of Web of Science,Cochrane Library,PubMed,Embase,VIP database,WanFang Data,CNKI,and China Biomedical Literature Database were conducted from inception to May 31,2022.Two researchers screened the literatures according to inclusion and exclusion criteria,extracted basic information and data of the included literatures,and evaluated the risk of bias of the literatures.Meta-analysis was performed using RevMan5.4 software.Results A total of 10 articles were included,involving 529 patients with temporomandibular arthritis caused by JIA.The results of the meta-analysis showed that the functional appliance could relieve the maxillofacial pain in patients with JIA(OR=0.24,95%CI:0.14-0.42,P<0.05),improve the maximal mouth opening(MD=11.73,95%CI:8.78-14.69,P<0.05),and reduce the click of temporomandibular joint(OR=0.32,95%CI:0.18-0.58,P<0.05).Conclusion Functional appliance can effectively improve the maxillofacial symptoms of temporomandibular arthritis caused by JIA,which is worthy of clinical promotion.
