加味封髓汤对胆热脾虚型复发性口腔溃疡口腔微生态菌群和细胞免疫功能的影响

摘　　要：目的 研究加味封髓汤对胆热脾虚型复发性口腔溃疡口腔微生态菌群和细胞免疫功能的影响。方法 以2020年4月-2021年5月口腔科门诊治疗的138例胆热脾虚型复发性口腔溃疡患者作为本次研究对象,全部患者按照分层抽样法随机分为观察组和对照组各69例,对照组患者给予复合维生素结合胸腺肽治疗,观察组在对照组基础上给予加味封髓汤治疗。对比治疗效果,治疗前后口腔内菌群水平及细胞免疫功能。结果 观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。两组治疗后口腔内链球菌、韦荣氏菌定量明显高于治疗前,观察组口腔内链球菌、韦荣氏菌定量明显高于对照组,差异均有统计学意义(P<0.05);两组口腔奈瑟菌定量与治疗前相比差异无统计学意义(P>0.05)。两组治疗后血清CD_(3)^(+)、CD_(4)^(+)、CD_(56)^(+)明显高于治疗前,观察组CD_(3)^(+)、CD_(4)^(+)、CD_(56)^(+)明显高于对照组,差异均有统计学意义(P<0.05)。结论 加味封髓汤治疗胆热脾虚型复发性口腔溃疡可以显著提高治疗效果,改善口腔微生态菌群水平,提高细胞免疫功能,治疗效果显著,值得临床推广应用。Objective To study the effect of(加味封髓汤)on oral microflora and cellular immune function of recurrent oral ulcer with gallbladder heat and spleen deficiency.Methods From April 2020 to May 2021,138 patients with recurrent oral ulcer of gallbladder heat and spleen deficiency type treated in the outpatient department of stomatology of the hospital were taken as the research object.All patients were randomly divided into observation group(69 cases)and control group(69 cases)according to stratified sampling method.Patients in the control group were treated with multivitamins combined with thymosin,while those in the observation group were treated with Jiawei Fengsui Decoction on the basis of the control group.The therapeutic effect,the oral flora level and cellular immune function before and after treatment were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the amounts of Streptococcus and Veronese in the oral cavity of the two groups were significantly higher than those before treatment,and the amounts of Streptococcus and Veronese in the oral cavity of the observation group were significantly higher than those of the control group(P<0.05).There was no significant difference in oral Neisseria quantitative of the two groups compared with those before treatment(P>0.05).After treatment,the serum levels of CD_(3)^(+),CD_(4)^(+) and CD_(56)^(+) in the two groups were significantly higher than those before treatment,while those in the observation group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Jiawei Fengsui Decoction can significantly improve the therapeutic effect and the level of oral microecological flora,and enhance the cellular immune function in patients with recurrent oral ulcer due to gallbladder heat and spleen deficiency,which is worthy of clinical application.
