摘　　要：目的观察经巩膜睫状体光凝(TCP)治疗恶性青光眼的短期与中期有效性和安全性。方法收集2020年1月至2022年12月青岛市城阳区人民医院收治的100恶性青光眼患者进行前瞻性研究,按照随机数字表法将其分为观察组(n=50)与对照组(n=50),分别应用TCP治疗与睫状体冷冻术。比较两组患者手术前后眼压、眼球疼痛程度、视力变化情况,随访术后并发症发生情况。结果术后1 d、1周、1个月、3个月、6个月,观察组患者眼压均明显低于对照组,差异均有统计学意义(P<0.05)。术后1 d、1周、1个月、3个月,观察组患者的眼球疼痛程度评分均明显低于对照组,差异均有统计学意义(P<0.05),术后6个月,两组患者的眼球疼痛程度评分比较,差异无统计学意义(P>0.05)。术后1周、3个月、6个月,观察组患者的视力指标水平均明显优于对照组,差异均有统计学意义(P<0.05)。观察组患者的并发症发生率为6.00%,明显低于对照组(20.00%),差异有统计学意义(P<0.05)。结论与睫状体冷冻术相比,TCP治疗恶性青光眼,有利于改善患者眼压及视力,减轻眼球疼痛程度,术后并发症少,短期及中期疗效确切。Objective To observe the efficacy and safety of transscleral cyclops photocoagulation(TCP)in the treatment of malignant glaucoma in short and medium term.Methods From January 2020 to December 2022,100 malignant glaucoma patients in Qingdao Chengyang District People's Hospital were collected for a prospective cohort study.The patients were divided into observation group(n=50)and control group(n=50)according to the random number table method.TCP treatment and ciliary body cryosurgery were applied respectively.The intraocular pressure,degree of eyeball pain,and changes in vision between the two groups before and after surgery were compared,and the incidence of postoperative complicationswas follow up.Results At 1 day,1 week,1 month,3 months,and 6 months after surgery,the intraocular pressure in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).At first day,one week,one month,and three months after surgery,the scores of eyeball pain in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).At sixth month after surgery,there was no statistically significant difference in the score of eyeball pain between the two groups(P>0.05).At 1 week,3 months,and 6 months after surgery,the visual acuity index levels of the patients in the observation group were significantly better than those in the control group,the differences were statistically significant(P<0.05).The incidence of complications in the observation group was 6.00%,which was significantly lower than that in the control group(20.00%),the difference was statistically significant(P<0.05).Conclusion Compared with ciliary body cryosurgery,TCP in the treatment of malignant glaucoma is beneficial for improving the patient's intraocular pressure and vision,reducing the degree of ocular pain,reducing postoperative complications,and providing reliable short-term and medium-term results.
