摘　　要：目的探究高危型人乳头瘤病毒(HR-HPV)、液基薄层细胞学(TCT)联合检测对宫颈癌与癌前病变的诊断价值。方法选取2021年1月至2022年1月九江市第三人民医院收治的96例宫颈病变患者作为研究对象,所有患者均接受阴道镜下活检、HR-HPV联合TCT检测,统计所有患者阴道镜活检情况,以阴道镜下活检结果为“金标准”,分析HR-HPV联合TCT检测对宫颈癌与癌前病变的诊断价值。结果经阴道镜活检结果显示,96例宫颈病变患者中癌前病变76例,占比79.17%(76/96),宫颈癌20例,占比20.83%(20/96);HR-HPV联合TCT鉴别诊断宫颈癌与癌前病变的阳性预测值、灵敏度、准确性、阴性预测值及特异度均高于单一HR-HPV、TCT诊断,差异有统计学意义(P<0.05)。结论宫颈癌与癌前病变采用HR-HPV联合TCT检测鉴别诊断价值满意,有较高的阳性预测值、灵敏度、准确性、阴性预测值及特异度。Objective To explore the diagnostic value of combined detection of high-risk human papillomavirus(HR-HPV)and liquid-based thin layer cytology(TCT)in cervical cancer and precancerous lesions.Methods A total of 96 patients with cervical lesions admitted to the Third People's Hospital of Jiujiang City from January 2021 to January 2022 were selected as the study objects.All patients received colposcopic biopsy and HR-HPV combined TCT detection.The colposcopic biopsy results of all patients were statistically analyzed,and the results of colposcopic biopsy were taken as the"gold standard".To analyze the diagnostic value of HR-HPV combined with TCT in cervical cancer and precancerous lesions.Results The results of transcolposcopic biopsy showed that among the 96 patients with cervical lesions,76 cases were precancerous lesions,accounting for 79.17%(76/96),and 20 cases were cervical cancer,accounting for 20.83%(20/96).The positive predictive value,sensitivity,accuracy,negative predictive value and specificity of HR-HPV combined with TCT in differential diagnosis of cervical cancer and precancerous lesions were higher than those of single HR-HPV and TCT diagnosis,and the differences were statistically significant(P<0.05).Conclusion HR-HPV combined with TCT is of satisfactory value in the differential diagnosis of cervical cancer and precancerous lesions,with high positive predictive value,sensitivity,accuracy,negative predictive value and specificity.
