半夏泻心汤加减治疗胃食管反流病68例临床研究

摘　　要：目的探讨半夏泻心汤加减治疗在胃食管反流病的临床效果及对中医证候积分、血清炎性因子、生活质量等的影响。方法选取2019年3月—2021年3月西安交通大学医院中医科及西安市中医医院脾胃科收治的胃食管反流病患者68例作为研究对象,根据随机数字表法按1∶1将患者分为观察组(n=34)及对照组(n=34)。其中对照组给予奥美拉唑联合铝碳酸镁方案治疗,观察组在对照组基础上给予半夏泻心汤加减治疗,两组均持续治疗8周。比较治疗前后两组中医证候积分、血清炎性因子及生活质量的改善效果,并对其疗效及安全性进行记录。结果治疗前两组中医证候积分、血清白细胞介素6(IL-6)、肿瘤细胞坏死因子(TNF-α)、超敏C反应蛋白(hs-CRP)水平及胃食管反流病相关生活质量量表(GERD-HRQL)积分比较无明显差异(P>0.05),治疗后均较治疗前有改善(P<0.05),且观察组改善效果优于对照组(P<0.05);观察组临床总有效率[94.12%(32/34)]高于对照组[76.47%(26/34)],差异有统计学意义(P<0.05);随访调查发现观察组不良反应发生率低于对照组,但差异无统计学意义(P>0.05)。结论半夏泻心汤加减治疗用于胃食管反流病中,不仅能有效缓解其反流症状,还能降低血清炎症因子水平,抑制炎症反应,对于提高生活质量也具有重要作用,相比于常规治疗而言其临床疗效更显著,安全性也更好。Objective To investigate the clinical effect of Modified Banxia Xiexin Decoction(半夏泻心汤加减)in the treatment of gastroesophageal reflux disease,and the influence on TCM syndrome scores,serum inflammatory factors and quality of life.Methods From March 2019 to March 2021,68 patients with gastroesophageal reflux disease who were admitted to the Department of Traditional Chinese Medicine of Xi'an Jiaotong University Hospital and the Department of Spleen and Stomach of Xi'an Traditional Chinese Medicine Hospital were selected as the research objects.They were divided into observation group(34 cases)and control group(34 cases).On this basis,the patients in the observation group were treated with Modified Banxia Xiexin Decoction.Both groups were given 8 weeks of treatment.TCM syndrome scores,serum inflammatory factors and quality of life of the two groups were compared before and after treatment.The curative effect and safety were recorded.Results There was no statistically significant difference in TCM syndrome scores,levels of serum interleukin-6(IL-6),tumor cell necrosis factor(TNF-α)and high-sensitivity C-reactive protein(hs-CRP)and the gastroesophageal reflux disease-health related quality of life(GERD-HRQL)scores between the two groups before treatment(P>0.05).After treatment,these indicators were improved(P<0.05),and the improvement effect of the observation group was better than that of the control group(P<0.05).The total clinical response rate of the observation group[94.12%(32/34)]was higher than that of the control group[76.47%(26/34)](P<0.05).Follow-up investigation found that the incidence of adverse reactions in the observation group was lower than that in the control group(P>0.05).ConclusionThe treatment of modified Banxia Xiexin Decoction for gastroesophageal reflux disease can not only effectively relieve the symptoms but also reduce the levels of serum inflammatory factors,inhibit the inflammatory response and improving the quality of life.Compared with the conventional treatment,it is more
