养血柔肝针法联合耳穴压豆治疗脑卒中后睡眠障碍(肝血亏虚)患者对睡眠质量及脑血流的影响

摘　　要：目的探究养血柔肝针法联合耳穴压豆对肝血亏虚型脑卒中后睡眠障碍(PSSD)患者睡眠质量及脑血流的影响。方法选择医院2019年1月—2021年12月收治的PSSD患者100例,按照简单随机化分为观察组和常规组,各50例,比较两组中医证候积分、睡眠质量、脑血流及不良反应。结果治疗前,两组中医证候积分、匹兹堡睡眠质量指数量(PSQI)分值、阿森斯失眠量表(AIS)分值、椎动脉收缩峰血流速度(Vp)、基底动脉Vp差异无统计学意义(P>0.05);治疗2周后,两组中医证候积分、PSQI分值、AIS分值均降低,观察组低于常规组(P<0.05),椎动脉Vp、基底动脉Vp均升高,观察组高于常规组(P<0.05)。两组头晕头痛、口干、恶心呕吐、嗜睡、腹泻等不良反应差异无统计学意义(P>0.05)。结论养血柔肝针法联合耳穴压豆可以有效改善PSSD患者睡眠质量和脑血流,安全性良好。Objective To explore the effects of nourishing blood and softening liver acupuncture combined with auricular plaster therapy on sleep quality and cerebral blood flow of patients with post-stroke sleep disorders(PSSD)of liver-blood deficiency type.Methods A total of 100 patients with PSSD were admitted to the hospital between January 2019 and December 2021.According to simple randomization method,the patients were divided into observation group and routine group,50 cases in each group.The scores of TCM syndromes,sleep quality,cerebral blood flow and adverse reactions were compared between the two groups.Results Before treatment,there was no significant difference in scores of TCM syndromes,Pittsburgh Sleep Quality Index(PSQI),Athens Insomnia Scale(AIS),peak systolic velocity of vertebral artery(Vp)or basilar artery Vp between the two groups(P>0.05).After 2 weeks of treatment,the scores of TCM syndromes,PSQI and AIS in both groups were decreased,which were lower in the observation group than those in the routine group(P<0.05).After 2 weeks of treatment,Vp values of vertebral artery and basilar artery in both groups were increased,which were higher in the observation group than those in the routine group(P<0.05).There was no significant difference in adverse reactions(dizziness and headache,dry mouth,nausea and vomiting,lethargy,diarrhea)between the two groups(P>0.05).Conclusion Nourishing blood and softening liver acupuncture combined with auricular plaster therapy can effectively improve sleep quality and cerebral blood flow in PSSD patients,with good safety.
