温针灸配合穴位敷贴治疗乳腺癌根治术后上肢水肿临床效果观察

摘　　要：目的探讨温针灸配合穴位敷贴治疗乳腺癌根治术后上肢水肿的临床效果。方法将2016年1月—2021年2月医院收治的150例乳腺癌根治术后上肢水肿患者简单随机分为对照组(73例)和观察组(77例),两组患者均给予常规治疗,对照组在上述基础给予温针灸治疗,观察组在对照组的基础上配合穴位敷贴治疗,两组患者均连续治疗1个月,比较两组患者的临床疗效、中医证候积分、疼痛情况、卡氏功能状态评分(KPS),统计并发症发生情况。结果观察组临床有效率(92.21%,71/77)较对照组(73.97%,54/73)显著升高,且差异有统计学意义(P<0.05);治疗前,两组患者上肢水肿、肢体麻木、酸痛、乏力积分比较差异无统计学意义(P>0.05),治疗后,两组患者上述中医证候积分均下降,且观察组低于对照组(P<0.05);治疗前,两组患者视觉模拟评分(VAS)、KPS评分比较无统计学意义(P>0.05),治疗后,两组患者VAS评分下降,KPS评分升高,且观察组优于对照组(P<0.05);两组患者均未出现明显不良反应。结论乳腺癌根治术后上肢水肿患者应用温针灸配合穴位敷贴治疗,临床效果佳,可有效改善临床症状,减轻患者疼痛,促进患肢功能恢复,且安全性高。Objective To investigate the clinical effect of warming needle moxibustion combined with acupoint application in treating upper limb edema after radical mastectomy.Methods A total of 150 patients who developed upper limb edema after radical mastectomy and admitted to the hospital between January 2016 and February 2021 were randomly divided into control group(73 cases)and observation group(77 cases).Both groups were given conventional treatments.On this basis,the patients in the control group were treated with warm needling moxibustion and those in the observation group were treated with warm needling moxibustion combined with acupoint application.All underwent 1 month of continuous treatment.The clinical effects,TCM syndrome scores,visual analogue scale(VAS)scores and Karnofsky Perfomance Status(KPS)scores of the two groups were compared.The occurrence of complications was counted.Results The clinical response rate of the observation group(92.21%,71/77)was significantly higher than that of the control group(73.97%,54/73)(P<0.05).The scores of upper limb edema,limb numbness,soreness and fatigue showed no statistically significant difference between the two groups before treatment(P>0.05).After treatment,the above-mentioned TCM symptom scores of the two groups were decreased,and the observation group had lower scores than the control group(P<0.05).There was no statistically significant difference between the two groups in VAS score or KPS score before treatment(P>0.05).After treatment,the VAS scores of the two groups were decreased,and the KPS scores were increased.The observation group had better scores than the control group(P<0.05).No obvious adverse reactions were observed in the two groups.Conclusion Warm needling moxibustion combined with acupoint application is effective in the treatment of patients with upper limb edema after radical mastectomy,which can effectively improve the clinical symptoms,relieve pain and promote the recovery of affected limb function,with high safety.
