半夏柴桂泻心汤联合西药治疗幽门螺杆菌感染的慢性胃炎脾胃湿热证临床研究

摘　　要：目的观察半夏柴桂泻心汤联合西药治疗幽门螺杆菌(Hp)感染的慢性胃炎脾胃湿热证的效果。方法将100例Hp感染的慢性胃炎脾胃湿热证患者采用随机数字表法分为2组。对照组50例予铋剂四联方案常规治疗,治疗组50例在对照组治疗基础上加半夏柴桂泻心汤治疗,2组均治疗14天。比较2组治疗结束后1个月Hp根除率及治疗结束后3个月复发率,比较2组治疗前后中医症状评分、胃肠疾病生命质量指数(GIQLI)量表评分,统计2组治疗期间不良反应发生率。结果治疗结束后1个月,治疗组Hp根除率94.0%(47/50),对照组Hp根除率78.0%(39/50),治疗组根除率高于对照组(P<0.05)。治疗后,2组各单项中医症状评分及总分均较本组治疗前降低(P<0.05),且治疗组均低于对照组(P<0.05)。治疗后,2组GIQLI量表评分均较本组治疗前升高(P<0.05),且治疗组高于对照组(P<0.05)。治疗组不良反应发生率6%(3/50),对照组不良反应发生率36%(18/50),治疗组低于对照组(P<0.05)。治疗组复发率6.4%(3/47),对照组复发率23.1%(9/39),治疗组复发率低于对照组(P<0.05)。结论半夏柴桂泻心汤联合西药治疗Hp感染的慢性胃炎脾胃湿热证Hp根除效果较好,复发率低,不良反应少,能够有效改善患者症状和生活质量。Objective To observe the effect of Banxia Chaigui Xiexin Decoction combined with Western medicine for Helicobacter pylori(Hp)-induced chronic gastritis of spleen-stomach damp-heat syndrome.Methods A total of 100 patients with Hp-induced chronic gastritis(spleen-stomach damp-heat syndrome)were randomly assigned into the treatment group(n=50)and the control group(n=50).All patients were managed by bismuth quadruple therapy,and those in the treatment group were additionally treated with Banxia Chaigui Xiexin Decoction.Treated for 14 days,aiming to compare Hp eradication rate at 1 month after treatment,recurrence rate at 3 months after treatment,traditional Chinese medicine(TCM)syndrome score,Gastrointestinal Quality of Life Index(GIQLI).The incidence of adverse reactions was assessed.Results At 1 month after treatment,Hp eradication rate was 94.0%(47/50)in the treatment group,higher than that of 78.0%(39/50)in the control group(P<0.05).After treatment,TCM symptom scores in groups were decreased(P<0.05),which decreased in the treatment group compared with the control group(P<0.05).GIQLI scores in groups were increased(P<0.05),the treatment group was more prevalent(P<0.05).The incidence of adverse reactions and recurrence rate in the treatment group were lower than those in the control group(6%[3/50]vs 36%[18/50],6.4%[3/47]vs 23.1%[9/39],[all P<0.05],respectively).Conclusion For patients with Hp-induced chronic gastritis(spleen-stomach damp-heat syndrome),Banxia Chaigui Xiexin Decoction combined with Western medicine has good Hp eradication effect,which can effectively improve patients'symptoms and quality of life,with low recurrence rate and less adverse reactions.
