正畸正颌联合治疗唇腭裂术后牙颌面畸形的效果分析

摘　　要：目的:探讨正畸正颌联合治疗唇腭裂术后牙颌面畸形的临床效果。方法:选取2020年1月—2022年8月成武县人民医院(新院区)收治的唇腭裂术后牙颌面畸形患儿96例作为研究对象,按照随机数字表法分为观察组和对照组,各48例。对照组实施正颌手术治疗,观察组实施正畸正颌联合治疗。比较两组头影测量数据、言语功能、咀嚼功能。结果:治疗前,两组上牙槽座角(SNA)、下牙槽座角(SNB)、上下齿槽座角(ANB)比较,差异无统计学意义(P>0.05);治疗后,两组SNA、ANB高于治疗前,且观察组高于对照组,两组SNB低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗前,两组言语功能、咀嚼功能评分比较,差异无统计学意义(P>0.05);治疗后,两组言语功能、咀嚼功能评分高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论:正颌正畸联合治疗能够修复唇腭裂术后患儿的牙颌面畸形,改善患儿的言语功能和咀嚼功能。Objective:To explore the clinical effect of orthodontic-orthognathic combined treatment for dental and maxillofacialdeformities after cleft lip and palate surgery.Methods:A total of 96 children with dental and maxillofacial deformities after cleftlip and palate surgery who were diagnosed and treated in Chengwu County People's Hospital(New Campus) from January 2020 toAugust 2022 were selected as the study subjects.They were divided into control group and observation group according to randomnumber table method,with 48 cases in each group.The control group implemented orthognathic surgery treatment,and theobservation group implemented orthodontic-orthognathic combined treatment.The cephalometric data,speech function,andmasticatory function were compared between two groups.Results:Before treatment,there were no significant differences in sellanasion A point angle(SNA),sella nasion B point angle(SNB),and difference between angle SNB and SNA angle(ANB) betweenthe two groups(P>0.05).After treatment,the SNA and ANB in the two groups were higher than those before treatment,and theobservation group was higher than the control group;the SNB in the two groups was lower than that before treatment,and theobservation group was lower than the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in speech function and masticatory function between the two groups(P>0.05).After treatment,the scores of speech function and masticatory function in the two groups were higher than those before treatment,and theobservation group was higher than the control group,and the difference was statistically significant(P<0.05).Conclusion:Theorthodontic-orthognathic combined treatment can repair the dental and maxillofacial deformities of children after cleft lip andpalate surgery,and improve the speech function and masticatory function of children.
