基于SEER数据库的Lauren分型胃腺癌患者的术前全身性治疗疗效分析

摘　　要：目的:分析经术前治疗的Lauren分型胃腺癌患者的生存与预后之间的关系,为不同组织学类型的胃腺癌的治疗决策和随访策略提供依据。方法:回顾性分析SEER数据库中筛选的胃腺癌患者。观察其OS和CSS。对预后资料进行生存分析及预后影响因素分析。结果:PSM配对后5736例Lauren分型胃腺癌患者入组。生存分析结果:肠型胃腺癌患者、术前全身性治疗胃癌患者总生存及肿瘤特异性生存较好。COX多因素回归分析示:年龄、种族、原发部位、AJCC分期、T分期、N分期、化疗、放疗、术前全身性治疗、Lauren分型是影响胃腺癌患者预后的独立因素(P均<0.001)。结论:年龄、种族、原发部位、AJCC分期、T分期、N分期、化疗、放疗、术前全身性治疗、Lauren分型是影响胃癌患者生存的独立预后因素。术前治疗可为Lauren分型胃腺癌患者带来生存获益。Objective:To analyze the relationship between survival and prognosis of Lauren classification gastric adenocarcinoma patients who have undergone preoperative treatment,providing a basis for treatment decisions and follow-up strategies for different histological types of gastric adenocarcinoma.Method:Retrospective analysis of selected gastric adenocarcinoma patients in the SEER database was made to observe its OS and CSS and perform survival analysis and prognostic influencing factors analysis on prognostic data.Result:5736 Lauren classified gastric adenocarcinoma patients were enrolled after PSM pairing.Survival analysis results:Patients with intestinal type gastric adenocarcinoma and preoperative systemic treatment for gastric cancer had better overall survival and tumor specific survival.COX multivariate regression analysis showed that age,race,primary site,AJCC stage,T stage,N stage,chemotherapy,radiotherapy,preoperative systemic treatment,and Lauren classification were independent factors affecting the prognosis of gastric adenocarcinoma patients(P<0.001).Conclusion:Age,race,primary site,AJCC stage,T stage,N stage,chemotherapy,radiotherapy,preoperative systemic therapy,and Lauren classification are independent prognostic factors that affect the survival of gastric cancer patients.Preoperative treatment can bring survival benefits to patients with Lauren type gastric adenocarcinoma.
