摘 要:目的:观察女性精神分裂症患者血清性激素水平与认知功能的关系,探讨其在女性精神分裂症患者认知功能障碍中可能产生的影响。方法:选择某医院住院女性精神分裂症患者125例。测定促黄体生成素、卵泡刺激素、泌乳素、雌二醇、孕酮及睾酮的水平,用神经心理状态评定量表和Stroop色词测验评估认知功能。结果:女性精神分裂症患者血清性激素水平与Stroop色词测验存在相关性;E2与RBANS测验总分(r=0.196,P=0.030)、即刻记忆得分(r=0.202,P=0.025)、语言功能得分(r=0.179,P=0.048)呈显著正相关;LH与RBANS测验总分(r=-0.218,P=0.015)、语言功能得分呈显著负相关(r=-0.218,P=0.015);患者RBANS总分值与LH水平呈显著负相关(β=-0.291,P=0.032),与E2(β=0.034,P=0.037)、受教育年限呈显著正相关(β=2.248,P<0.001)。结论:下丘脑-垂体-性腺轴功能失调可能参与女性精神分裂症患者认知障碍的过程,E2可能是认知功能的保护因素,LH可能是认知功能的危险因素。Objective:To observe the relationship between serum sex hormone levels and cognitive function in female schizophrenic patients and to explore its possible influence on cognitive dysfunction in female schizophrenic patients.Methods:One hundred and twenty-five female schizophrenia patients who attended the psychiatric department of hospital were collected.The levels of luteinizing hormone,follicle stimulating hormone,prolactin,estradiol,progesterone and testosterone were measured,and the cognitive function of patients was assessed by the Rating Scale of Neuropsychological Status and Stroop Color Word Test.Results:There is a correlation between serum sex hormone level and Stroop color word test in female schizophrenia patients.E2 was significantly and positively correlated with total RBANS score(r=0.196,P=0.030),with immediate memory score(r=0.202,P=0.025)and with verbal function score(r=0.179,P=0.048);LH was significantly and negatively correlated with total RBANS score(r=-0.218,P=0.015)and verbal function scores(r=-0.218,P=0.015);with Total RBANS scores in female schizophrenic patients were significantly negatively correlated with LH levels(β=-0.291,P=0.032),significantly positively correlated with E2(β=0.034,P=0.037),and significantly positively correlated with years of education(β=2.248,P<0.001).Conclusion:Hypothalamic-pituitary-gonadal axis dysfunction may be involved in process of cognitive impairment in female schizophrenic patients,endogenous E2 may be a protective factor for cognitive function,and LH may be a risk factor for cognitive impairment.
