摘　　要：目的探讨右美托咪定静脉复合麻醉在玻璃体切割术中的安全性及对患者和医生满意度的影响。方法选取2022年1月至6月于新疆四七四医院拟行玻璃体切割术的患者269例,根据随机数字表法将其分成右美托咪定组(n=143)和生理盐水组(n=126)。比较两组患者的一般资料、血流动力学指标、Ramsay镇静评分、患者和手术医生对麻醉的满意度。结果右美托咪定组患者镇静恰当的比例显著高于生理盐水组(P<0.01)。右美托咪定组患者及手术医生对术中麻醉的满意度均显著高于生理盐水组(P<0.05)。手术开始后10min和30min、手术结束时,右美托咪定组患者的心率、呼吸频率、平均动脉压均显著低于同期生理盐水组(P<0.05),两组患者的血氧饱和度比较差异均无统计学意义(P>0.05)。两组患者的恶心、呕吐、呼吸抑制等不良反应发生率比较差异无统计学意义(P>0.05)。结论右美托咪定静脉复合麻醉是玻璃体切割术安全可靠的镇静方法,可提高患者和手术医生的满意度,无呼吸道并发症。Objective To investigate the safety of dexmedetomidine intravenous anesthesia in vitrectomy and the effect on the satisfaction of patients and doctors.Methods A total of 269 patients who were to undergo vitrectomy in Xinjiang 474 Hospital from January to June 2022 were selected and divided into dexmedetomidine group(n=143)and saline group(n=126)according to random number table method.The general data,hemodynamic parameters,Ramsay sedation score,and the satisfaction of patients and surgeons with anesthesia were compared between the two groups.Results The proportion of patients with proper sedation in dexmedetomidine group was significantly higher than that in saline group(P<0.01).The satisfaction of patients and surgeons in dexmedetomidine group was significantly higher than that in saline group(P<0.05).10 min and 30 min after operation and at the end of operation,the heart rate,respiratory rate,and mean arterial pressure in dexmedetomidine group were significantly lower than those in saline group(P<0.05),and there was no statistically significant difference in blood oxygen saturation between the two groups(P>0.05).There was no statistically significant difference in the incidence of adverse reactions such as nausea,vomiting,and respiratory depression between the two groups(P>0.05).Conclusion Dexmedetomidine intravenous anesthesia is a safe and reliable sedative method for vitrectomy,which can improve the satisfaction of patients and surgeons without respiratory complications.
