盐酸度洛西汀对晚期癌性疼痛患者情绪及单胺神经递质水平的影响

摘　　要：目的 分析晚期癌性疼痛患者应用盐酸度洛西汀对疼痛程度、不良心理状态及单胺神经递质水平的影响.方法 将2020年10月~2022年10月期间我院收治的86例晚期癌性疼痛患者作为研究对象,采用随机数字表法进行分组.对照组43例给予盐酸羟考酮治疗,实验组43例在此基础上增加盐酸度洛西汀治疗.4周后,对比两组患者疼痛程度、不良心理状态、单胺神经递质水平、不良反应.结果 实验组患者疼痛视觉模拟量表(VAS)评分、简明疼痛评估量表(BPI)评分分值均低于对照组(P<0.05);实验组患者汉密尔顿抑郁量表(HAMD)评分、汉密尔顿焦虑量表(HAMA)评分分值均低于对照组(P<0.05);实验组患者去甲肾上腺素(NE)、5-羟色胺(5-HT)、多巴胺(DA)水平均高于对照组(P<0.05);两组患者不良反应发生率无明显差异(P>0.05).结论 盐酸度洛西汀能够有效减轻晚期癌性疼痛患者疼痛程度,改善不良心理状态及单胺神经递质水平,且安全性高.Objective To analyze the effects of duloxetine hydrochloride on pain degree,adverse mental state and monoamine neurotransmitter level in patients with advanced cancer pain.Methods A total of 86 patients with ad-vanced cancer pain admitted to our hospital from October 2020 to October 2022 were selected as research objects and grouped by random number table method.Forty-three patients in the control group were treated with oxycodone hydro-chloride,and 43 patients in the experimental group were treated with duloxetine hydrochloride.After 4 weeks,pain degree,adverse psychological state,monoamine neurotransmitter level and adverse reactions were compared between the two groups.Results The scores of visual Analog Scale(VAS)and Concise Pain Assessment Scale(BPI)in ex-perimental group were lower than those in control group(P<0.05).The scores of Hamilton Depression Scale(HAMD)and Hamilton Anxiety Scale(HAMA)in the experimental group were lower than those in the control group(P<0.05).The levels of norepinephrine(NE),5-hydroxytryptamine(5-HT)and dopamine(DA)in experimental group were higher than those in control group(P<0.05).The incidence of adverse reactions was compared between 2 groups(P>0.05).Conclusion Duloxetine hydrochloride can effectively reduce the pain degree of patients with advanced cancer pain,improve the adverse mental state and the level of monoamine neurotransmitter,with high safety.
