摘　　要：目的 分析维持性血液透析(MHD)老年患者睡眠障碍与抑郁焦虑情绪、营养状况及微炎症的关系.方法 选取行MHD的患者120例为研究对象.采用问卷形式收集患者的一般资料并通过电子病历库收集其早期的临床资料,其中2例于研究过程中脱离,将纳入的118例患者根据是否存在睡眠障碍分为睡眠障碍组(n=70)、非睡眠障碍组(n=48)两组,分析MHD老年患者的睡眠质量与其负面情绪、营养状况及微炎症的关系.结果 睡眠障碍组的焦虑自评量表(SAS)与抑郁自评量表(SDS)、改良主观综合评估法(MQSGA)评分及甲状旁腺激素(PTH)、C-反应蛋白(CRP)水平均高于非睡眠障碍组,血清白蛋白(ALB)水平低于非睡眠障碍组(P<0.05);Pearson相关分析结果显示,SAS评分、SDS评分、MQSGA评分及PTH、CRP水平与老年MHD患者睡眠质量呈正相关关系,ALB与老年MHD患者睡眠质量呈负相关关系(P<0.05).结论 抑郁、焦虑情绪、营养状况低下及微炎症状态与MHD老年患者睡眠障碍的发生密切相关.Objective To analyze the relationship between sleep disorders,depression,anxiety,nutritional status,and micro inflammation in elderly patients undergoing maintenance hemodialysis(MHD).Methods 118 pa-tients who underwent MHD were selected as the study subjects.The general data of patients were collected in the form of questionnaires and their early clinical data were collected through the electronic medical record database.Two of them were separated during the study.118 patients were divided into sleep disorder group(n=70)and non sleep disorder group(n=48)according to whether there was sleep disorder.The sleep quality of elderly MHD patients and their nega-tive emotions were analyzed.Results The scores of SAS,SDS,MQSGA and the levels of parathyroid hormone(PTH)and C-reactive protein(CRP)in the sleep disorder group were higher than those in the non sleep disorder group,and the levels of serum albumin(ALB)were lower than those in the non sleep disorder group,with significant differences(P<0.05).Pearson correlation analysis showed that SAS score,SDS score,MQSGA score,PTH and CRP levels were positively correlated with the sleep quality of elderly MHD patients,while ALB was negatively correlated with the sleep quality of elderly MHD patients,with a statistically significant difference(P<0.05).Conclusion Severe de-pression,anxiety,low nutritional status,and micro inflammatory status are closely related to the occurrence of sleep disorders in elderly patients with MHD.
