吡柔比星辅助立体定向分割放疗对原发性肝癌患者肝功能及免疫功能的影响

摘　　要：目的探讨吡柔比星辅助立体定向分割放疗治疗原发性肝癌的临床疗效,分析其对肝功能、免疫功能的影响。方法选取2020年1月—2022年3月濮阳市油田总医院收治的107例原发性肝癌患者为研究对象,随机分为对照组53例、研究组54例。对照组予以立体定向分割放疗+顺铂治疗,研究组予以立体定向分割放疗+吡柔比星治疗。比较两组临床疗效及治疗前、治疗1个周期、治疗2个周期后肝功能指标[总胆红素(TBIL)、直接胆红素(DBIL)、丙氨酸氨基转移酶(ALT)、白蛋白]、免疫功能(CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+))、肿瘤标志物[糖类抗原125(CA125)、癌胚抗原(CEA)、甲胎蛋白(AFP)]水平及微小RNA(miR-495-3p)、葡萄糖调节蛋白78(GRP78)表达量。观察两组治疗期间不良反应发生情况。结果研究组有效率87.04%、控制率94.44%分别高于对照组的69.81%、79.25%(P<0.05);治疗1个周期、2个周期后研究组血清TBIL、DBIL、ALT、CA125、CEA、AFP水平低于对照组,白蛋白水平高于对照组(P<0.05);治疗1个周期、2个周期后研究组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)高于对照组(P<0.05);治疗1个周期、2个周期后研究组血清miR-495-3p表达量高于对照组,GRP78 mRNA表达量低于对照组(P<0.05);两组不良反应比较差异无统计学意义(P>0.05)。结论吡柔比星辅助立体定向分割放疗治疗原发性肝癌,可提高临床疗效,改善患者肝功能、免疫功能,抑制肿瘤标志物表达,且具有一定安全性。Objective To investigate the clinical efficacy of Pirarubicin assisted stereotactic segmentation radiotherapy in the treatment of primary liver cancer,and to analyze its effect on liver function and immune function.Methods A total of 107 patients with primary liver cancer admitted to our hospital from January 2020 to March 2022 were selected as research objects and were randomly divided into control group(53 cases)and study group(54 cases).The control group was treated with stereotactic fractionated radiotherapy+cisplatin,while the study group was treated with stereotactic fractionated radiotherapy+pirarubicin.The clinical efficacy,liver function indexes[total bilirubin(TBIL),direct bilirubin(DBIL),alanine aminotransferase(ALT),albumin],immune function(CD3^(+),CD4^(+),CD4^(+)/CD8^(+)),tumor markers[carbohydrate antigen 125(CA125)]were compared between the two groups before treatment,1 cycle and 2 cycles after treatment,carcinoembryonic antigen(CEA),alpha-fetoprotein(AFP),and micrornas(miR-495-3p)and glucose-regulatory protein 78(GRP78).The occurrence of toxic and side effects in the two groups during treatment was observed.Results The effective rate and control rate of the study group were 87.04%and 94.44%,respectively,higher than those of the control group(69.81%and 79.25%,P<0.05).The serum levels of TBIL,DBIL,ALT,CA125,CEA and AFP in the research group were lower than those of the control group after one and two cycles of treatment,while the albumin level was higher than that of the control group(P<0.05).CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the study group were higher than those of the control group after 1 and 2 cycles of treatment(P<0.05).After one and two cycles of treatment,the serum miR-495-3p expression level of the study group was higher than that of the control group,and the GRP78 mRNA expression level was lower than that of the control group(P<0.05).There was no significant difference in toxicity and side effects between the two groups(P>0.05).Conclusion Pirarubicin assisted stereotactic segmentation radi
